Summary:

The research goal is to study the function of kidneys and serum concentration of immunoregulatory cytokines (IL-1ГА, IL-6, IL-8, IL-10) in conditions of thyroid hormone deficit. Methods: The character and severity of renal dysfunction were investigated in patients with primary hypothyroidism. The clinical examination included study of serum concentration of creatinine, urea level, potassium and natrium in blood serum, blood and urine analyses, total cholesterol, high and low density lipoproteins, daily microalbuminuria, the performing of Zimnitsky test, urinary excretion of chemokines MCP-1, RANTES, rate calculation of glomerular filtration, thyroid hormone state. Results: The renal function in clinical hypothyroidism without concomitant kidney disturbances was characterized by normal parameters of the concentra-tional kidney function, reduction of glomerular filtration rate, increased of serum creatinine, urea excretion level of chemokine MCP-1 and level of cytokines IL-6, IL-8 in patients with autoimmune hypothyroidism were revealed. Conclusion: It was revealed that autoimmune hypothyroidism effected balance disturbance of cytokine-producing activity of Th1 and Th2 type therefore developing autoimmune state and disease progression

Objective of the study: determination of the degree of influence of bilirubin in the blood during obstructive jaundice, on blood clotting. Methods. A retrospective study of case histories of patients with obstructive jaundice who have been treated at the Regional Hospital of Saratov in the period from 2000 to 2010. Results. The results confirm the assumption that the causes of bleeding in obstructive jaundice is hepatic failure. Conclusion. Absence of bile in the small intestine in obstructive jaundice is not the cause of bleeding. Bile acids are not involved in metabolizing fat-soluble vitamin K1

Summary:

The aim of the study was to evaluate the parameters and comparison of wound healing in patients with diabetic foot syndrome using modern dressings and traditional scheme of local treatment of chronic wounds. Materials and methods: Clinical part of the work has been based on the results of a comprehensive investigation of the parameters of wound healing in 154 diabetic foot patients. Results: Optimum results are obtained by treatment of wound infection in patients with neuropathic diabetic foot shape using highly technological dressings. Conclusion: The modern interactive dressings may actively interfere to all phases of wound healing in patients with purulent-necrotic complications of diabetes mellitus

Index of early renal damage in children with vesicoureteral reflux

Summary:

Objective: To determine sensitive and objective criteria of nephrosclerosis by testing the levels of markers of inflammation (MCP-1), fibrogenesis (TGF-(31), renal parenchymal damage (collagen type IV, a-GST, тт-GST), angiogenesis (VEGF) in urine of children with vesicoureteral reflux (VUR). Materials and methods: 80 patients with different stages of VUR have been observed before and 6 months after treatment. Results: «Index of early renal damage» can be found as correlation between VEGF and collagen type IV concentration. Conclusion: Estimation of «index of early renal damage» could be included in pediatric practice for the control of urinary tract dysfunction in children with VUR

Objective: To improve the efficiency of surgical rehabilitation of patients with traumas, their effects. Materials and methods: Short-term and follow-up results of the surgical treatment of patients with traumas and their effects have been analyzed. Statistical research methods have been used. Results: the efficiency of medical technologies during the early rehabilitation of patients has been demonstrated. Conclusion: Adoption of the most efficient medical technologies of general surgical treatment and postoperative rehabilitation of patients with traumas and their effects in daily practice provides high-grade restoration of the extremity's function, shortening of treatment period, decrease in number of complications and invalidism

Summary:

Purpose. The purpose is to study the reaction of homeostasis system in stable and in distraction llizarov transosseous ostesysnthesis of the limbs bones at experimental animals (dogs) in highland conditions. Material and Methods. 63 adult mongrels aged from 1 to 3 years with 10-20 kg body weight and 15 cm minimal length of tibia were included in the experiment. Open osteosotmy of tibial mid-shaft followed by Kirschner wires insertion through tibial bone and llizarov frame assembly comprising 3 or 4 rings was performed under IV anaesthesia at 3200 m altitude above sea level. In experimental group including 33 mongrels the distraction was performed on the 5th day after osteotomy by 1 mm rate divided into 4 times within 10 days but in control group comprising 30 dogs the distraction was not provided. Every 10 days after distraction blood samples were taken in both groups to examine coagulation; the samples were taken from subcutaneous lateral tibial vein, ex tempora mixing with 3,8% solution of sodium citrate at 9:1 ratio. Then the plasma was separated and examined biochemically according to the accepted standards. Results. We obtained the data witnessing activation of coagulation component of homeostatic system in stable osteosynthesis of the limb bones using llizarov fixator with simultaneous inhibition of fibrinolytic component. In distraction osteosynthesis hyper-coagulation trend in homeostasis system with simultaneous suppression of fibrinolytic blood activity is vividly defined within the entire experiment period. Conclusion. Transosseous distraction osteosynthesis in highland condition causes considerable shifts in the system of homeostasis with definite domination of coagulation component which promotes formation of pre-thrombotic condition. Limb segment lengthening on the background of pre-thrombotic condition increases the risk of thromboembolic complications in highland conditions and requires appropriate measures of prophylaxis
