Endoscopic polypectomy in treatment of colon adenomatosis

Summary:

Objective: To study the value of endoscopic polypectomy in the treatment of family adenomatous polyposis of the colon. Materials and methods. A total of 7 patients with diffuse adenomatosis of the colon family, who were treated at the proctological department of Hospital № 8 in Saratov from 2004 to 2006 on the stage of diagnosis all patients underwent double-contrast irrigoscopy and fibrocolonoscopy with biopsy. Endoscopic removal of polyps performed by electroscission through fibrocolonoscope «Olympus CF-E3 L» with a standard loop diathermy and electrosurgical apparatus «Endothelial-1». Results. Total rehabilitation after endoscopic polyp from retained sections of the colon and adequate comprehensive preoperative patients are routinely carried out successfully three right-left-sided hemicolec-tomy and 4 on the diffuse polyposis of the colon, complicated by recurrent bleeding or malignancy of polyps. Conclusion. Non-radical surgery in combination with endoscopic polypectomy does not relieve the patient diffuse polyposis of the colon family of high risk of colorectal cancer. In the case of diffuse polyposis, endoscopic polypectomy family is of limited value, as not able to eliminate completely a potential substrate of the disease, which is the entire mucosa of the colon

Optimization of surgical treatment of diabetic foot syndrome in Saratov region

Summary:

The research goal is to estimate the results of multidisciplinary approach to treatment of diabetic foot syndrome in the conditions of specialized hospital centre. Materials and methods: The clinical part of work is based on the study of surgical assistance range, recurrence of infectious complications of diabetes mellitus and mortality rate in 226 diabetic foot syndrome patients. Optimum results of treatment have been received in a group of patients with neuropathic forms of lower extremities damage. Conclusion: Use of multidisciplinary approach to the treatment of patients with complications of diabetes mellitus has improved survival rate (x2=12,1; p=0,002) and quality of life of patients with diabetic foot syndrome in Saratov region

Specificity of intraabdominal endoprosthesis of umbilical and postoperative ventral hernias

Summary:

The research work objective was to examine the results of intraperitoneal plastics in the hernias of anterior abdominal wall. The experience of treatment of 89 patients with uncomplicated umbilical and postoperative ventral hernias was analized. The surgical treatment included an open intraperitoneal prosthetic hernioplasty. The hernial sac was not removed during the operation. The original techniques of treatment of hernia sac were used. Anatomical and morphological features in the structure of middle ventral hernias and their pathophysiological assessment were revealed. Practical recommendations for technology of intraperitoneal prosthesis were given. The early postoperative complications occurred: seroma (n=2), the outflow of serous fluid drainage for 5 days (n=1), infiltration of the umbilical area (n=3), suppuration of wounds (n=1). Vacuum drainage was performed in 24 patients after removal of large hernial defects. During the period from 6 months to 4 years recurrences were not revealed. The intraperitoneal surgery using a complex of musculo-aponeurotic tissues provided hernial implant fixation. Operation without the removal of the hernial sac reduced the trauma intervention. Method of suturing the surgical wound reduced the time of drainage and reduced the number of wound complications

Summary:

Purpose. Research objective — studying of consequences of the operated traumatic intracranial hematomas in the remote period. Material. The nearest and remote results of the operated traumatic intracranial hematomas at 105 patients in terms from 6 months till 3 years are analyzed. During research the anamnesis was studied, neurologic investigation, and also research cognitive functions by means of scale MMSE, the test of drawing of hours, a scale of studying of alarm/depression HADS, learning of 5 words, scale quality of life SF-36. Results. It is established, that in the remote period the condition of the majority of patients gradually improves, at the same time, frequent enough and expressed consequences which are necessary for analyzing with the purpose of optimization of outcomes and the forecast at the given disease are observed. The special attention should be given again developing complications to which it is possible to carry epileptic seizures and behavioral-memorable infringements. Conclusion. Studying of the remote consequences of this heavy kind of craniocereberal trauma allows to optimize results of treatment and to provide complex medical, labor, social and family adaptation

Summary:

Objective: The study of bone tissue repair in different conditions of stability of transosseous fixation (techniques of treatment) of injuries of bones of the shin. Material and methods: Analysis of treatment of 74 patients with unstable diaphyseal fractures of bones of the shin, with the use of different methods of osteosynthesis is carried out. Results: An optimal method of combined transosseous osteosynthesis for treatment of victims with unstable fractures of bones of the shin is offered. Conclusion: A complex approach to the decision of the available problems is offered, consisting in a combination of the use of different types of transosseous elements in the external fixation device which allows to improve results of treatment and to reduce disability period

Summary:

Objective: To study the opinion of citizens of the Russian federation (citizens of Saratov) according to population census. Materials and methods: There were interviewed 307 persons (residents of the city of Saratov). Sociological and statistical methods of research were used. Results: The most common way of participation in the census is filling rewriter questionnaire at home. The main reason for non-participation in the census is the insufficient number of enumerators. Conclusion: Saratov citizens have a positive attitude towards the census of the population and consider it as a necessary event. There was no single opinion revealed on the way of conducting of the next population census

Herpetic stomatitis development mechanism in patients with tonsillitis

Summary:

The research goal is to investigate mechanisms of formation and course of herpetic stomatitis in patients with focal chronic tonsillitis. It has been established that patients with stomatitis and tonsillitis are able to develop local inflammatory reaction. It is accompanied by increase of infectious severity and activation of anti-inflammatory cytokines in fluid of parodontal pockets. It has been also proved that the cause factor of frequent and long relapses of stomatitis in patients with tonsillitis is due to the development of pathological process as a result of disbalance of lipid peroxidation and antioxidant system, endotoxicosis syndrome.

Optimization of phonetic adaptation in patients after stomatological treatment as a way of medical care improvement

Summary:

Purpose: develop a methodology for optimizing the phonetic adaptation of patients after dental treatment. Materials and methods. A group of people after the orthopedic treatment or extensive restoration work with sound correction and without it has been studied. In order to correct sounds used technique of adaptive biocontrol with feedback. Results. The importance and effectiveness of adaptive biocontrol techniques with feedback in the optimization of phonetic adaptation. Conclusion. Method of phonetic adaptation optimization, which allows to reduce the rehabilitation period, and thus improves the quality of life in patients of dental profile.
