Diagnostics And Treatment Of Patients With Blood Circulation Insufficiency In Vertebrobasilar Bed

Summary:

For improvement of treatment results of patients with vertebrobasilar insufficiency we have analyzed data of medical examination and treatment of 182 patients stayed in clinics of Hospital Surgery and Nervous System Diseases from the 1st of January of 2005 till the 30th of June of 2009. Method of screening diagnostics, quantitative and qualitative blood flow estimation by means of Doppler ultrasonic investigation and angiography have been performed. The cause of blood circulation insufficiency in the vertebrobasilar bed at 87 patients was vertebral artery syndrome and in 95 cases there were the significant hemodynamical damages of blood flow in aorta arch branches and intracranial arteries. The neuroangiotropic therapy has been made for all patients, but the clinical improvement period was short in most cases, more prolonged effect was observed in addition of conservative therapy with plasmapheresis by indications. In the complex treatment of patients with vertebrobasilar insufficiency accompanied by degenerative diseases of spine chemical sympathectomy of vertebral artery in the III segment in the form of procaine and spirit-procaine blockades was made in 116 and 69 cases accordingly. The surgical sympathectomy was made in 15 patients. The surgical operations were made in 62 of 95 patients with significant hemodynamic pathology of blood flow in the vertebrobasilar region. In most cases isolative and combinative reconstructive surgery on carotid arteries was carried out. It played a significant role in blood flow correction. Favorable postoperative results were observed in most cases (93, 5%)

The article gives the detailed issue of results of complex inspection of 90 children and teenagers aged 4-17 with problems of chronic constipation, incontinence and encopresis. Ultrasonic screening has shown various pathology in the functional condition of arterial renal vessels. The method of biological feedback has been considered as prospective method of treatment excluding medication of bladder and small bowel dysfunction. The efficiency of the method was marked at combination of encopresis and incontinence. The purpose of the present research was the study of renal hemodynamics disturbances and working out methods of their correction. The examination included ultrasonic investigation, electromyography and uroflowmetry. The study of functional condition of anterior abdominal wall muscles and pelvic floor muscles was performed by means of «Miomed - 938». All patients received complex therapy on the basis of which the method of biological feedback was used

Summary:

One of the commonest urological complications of radical pelvic surgery is enuresis which effects patients quality of life and causes severe physical and moral sufferings. The operational methods of enuresis treatment are unproductive and often lead to recurring and more complicated surgical interventions. All this provides research and development of new non-invasive methods of treatment and rehabilitation of patients with enuresis. Treatment of enuresis after radical operations includes a system of gymnastic exercises for pelvic floor muscles and health resort factors and results in reduction of symptoms and opens new ways of rehabilitation of this category of patients.

Results Of Orthotopic Bladder Creation After Radical Cystectomy In Patients With Bladder Cancer

Summary:

This article presents the results of 116 radical cystectomies of patients with bladder cancer. In all the cases various generated modifications of orthotopic bladder including the authors' methodology have been carried out. The structure of complications during the post-operative period has been analyzed. Various complications occurred in 16,4 % of patients. In 3,4 % of cases they caused death. In 74% of cases direct dependence of postoperative complications on advanced tumorous process and severity of accomponied pathology has been identified. Direct dependence on the selection of method of urine derivation has not been found. Five-year cancer survival rate accounted 70 %. The study of patients' quality of life has been performed using EORTC-QLQ-C-30. High indices have been worked out. Dependence on the method of orthotopic bladder creation has been negligible

Summary:

Research objective is to study processes of lithogenesis at patients with considerably cured urolithiasis at androg-enous therapy. The patients observed in urological clinic of Omsk State Medical Academy have been included. By means of method «case-control» 2 groups have been formed : the basic group - 10 patients with considerably cured urolithiasis and with hypogonadism receiving androgenous therapy and the comparison group - 10 patients with considerably cured urolithiasis and with hypogonadism without receiving androgenous therapy. Processes of lithogenesis at patients who received androgenous therapy have proceeded statistically significantly in comparison with patients without receiving androgenous therapy. It has been proved that there is a negative influence of testosterone deficiency on processes of lithogenesis. The obtained data demonstrate that lithogenic processes at patients with hypogonadism are more expressed and lead to more frequent urolithiasis recurrence. Noninvasiveness, technical simplicity and availability of method «Litos-test» provides its use for express-screening method for inspection of the population and for estimation of antireccurent efficiency of androgen replacement therapy at men with urolithiasis
