Liver Functional State Changes At Epstein-Barr Virus Mononucleou-sis In Children

Summary:

The research presents the results of medical observation of 439 patients aged 1-18 with mononucleosis of Epstein-Barr Virus etiology. By means of instrumental and clinical laboratory methods of investigation the peculiarities of liver condition have been revealed in children of different age. The intensity of autosensibilization processes to the tissues of the organ has also been estimated. The results obtained demonstrate the interrelation of clinical, biochemical and instrumental signs of liver damage, as well as contribution of autoimmune processes to the development of organopa-thology at Epstein-Barr Virus infection

Peptic ulcer disease is one of the most widespread diseases. 6-10 % of adult population in Russia suffer from it. Demographic processes in the Russian Federation determine the increase of patients' number aged over 60 with peptic ulcer disease. It counts 10-35 % of all patients with this disease. A new methodological approach has been proposed according to the insufficient results of hypertension (HD) and atherosclerosis (AT) treatment and preventive measures. This method is characterized by the determination and taking into account the phases of systematic chronic inflammatory process in vessels and concentrates on the conduction of an active anti-inflammatory therapy (AIT). Special attention has been paid to the diagnostics and treatment of different phases of interseizure period (ISP), mainly of preactive phase. These measures should both prevent activation and progressing of the process and become a kind of prophylaxis

Clinical audit of medical care quality by computer informational analytical system in patients with hypertension followed in Saratov polyclinic

Summary:

The research goal is to point out potentiality of clinical audit of care quality carried out by IAS "Register AH" in hypertensive patients followed in Saratov polyclinic.
Data of medical cards of 205 hypertensive patients (average age 52,8±9,1 years) followed in Saratov polyclinic have been analyzed with the help of IAS "Register AH" indicators set. Implementation of clinical recommendations on diagnosis, treatment and following-up of hypertensive patients from 2007 till 2008 has been assessed.
There was no correspondence between statements of recommendations of diagnostic measures for clinical status, treatment and following-up of hypertensive patients. Clinical aim of treatment to support the principal blood pressure has not been achieved in 85% of patients. The main cause has consisted in the patient's clinical status without taking into account the treatment and dynamic follow-up.
Clinical audit has allowed the complex evaluation of medical care process in hypertensive patients followed in the studied polyclinic and allowed to reveal a cause of inappropriate results of care. The use of IAS "Register AH" systematizes medical data contained in ambulatory cards, automatizes the process of obtaining indicators results. It increases the reliability assessments

Endothelium Damage And Arterial Rigidity In Patients With Psoriatic Arthritis

Summary:

The research goal is to study the connection between activity of systemic inflammation, endothelium damage and arterial rigidity in patients with psoriatic arthritis (PsA). Expression of arterial damage (the level of circulation of endothe-liocytes (CEC)) and indices of arterial rigidity have been studied in 32 patients with PsA. Methods used in the investigation are oscillography, photopletismography and occlusion test. Control group consisted of 22 practically in good health state volunteers. Patients with PsA had endothelium dysfunction, damage of endothelium and higher arterial rigidity. These findings were related to PsA activity and observed in patients with serious joint damage

Summary:

The aim of this research is to increase effectiveness of treatment of patients in eclamptic coma. Current algorithm of intensive therapy corresponds to coma states methodology treatment of metabolic genesis accepted in neurological and neurosurgical practice. The main principal of intensive therapy tactics has been to restore volemic figures and perfusion-metabolic balance of brains. Decrease of mean arterial pressure (5-10% from the initial one) which defines the central perfusion pressure has been provided by magnesium sulphate and nimodipine. Stabizol has been used as plasma-substituting component to circulate in bloodstream till 36 hours. Nimodipine has provided increased cerebral
bloodflow. Proposed algorithm of intensive therapy has allowed to reduce lethality from 15.7% to 4.8%. Key words: eclamptic coma, cerebral bloodflow, cerebral perfusion pressure

Modern Technologies In Ectopic Pregnancy Diagnostics On Hospital Stage

Summary:

The present research provides analysis of results of preoperative examination of 680 patients with ectopic pregnancy depending on the range of surgical treatment. It has been shown that in case of progressive ectopic pregnancy the diagnostic significance of concentrations of human chorionic gonadotropin, transvaginal examination and laparos-copy depends on the duration of pregnancy. When the term of ectopic pregnancy is 3-4 weeks monitoring of p chorionic gonadotropin in blood serum (99.5%), transvaginal examination (58%) and laparoscopy (78.5%) are more informative. When the term of ectopic pregnancy is more than 4 weeks there is a high diagnostic value of monitoring p-subunit of chorionic gonadotropin (99.5%), transvaginal examination (68%) and laparoscopy (99,5%). The research has proved that pregnancy period of 3-4 weeks is optimal for laparoscopy and other operations
