Summary:

The aim: to search the possible ways for increase of individual occlusive splints»s protective properties. Material and methods: 42 occlusive splints have been made with reinforced protective characteristics by the original technique. Results. Study of the level of comfort of splints and estimation of periodontal tissues proved the need for individual splints with increased protective properties. Conclusion. The individual splints should be made with reinforced protective properties for the long-term use.

Summary:

Objective: to determine the level of dental health in children and adolescents living in the Saratov region. Material and methods. To assess the intensity and prevalence of major dental diseases 1510 patients have been examined by the method recommended by the WHO Collaborating Centre in Russia. Results. A comparative analysis of caries and occurrence of different types of teeth-maxillary anomalies among children aged from 3 to 18 years. Conclusion. The epidemiology of major dental diseases in the Saratov region is characterized by the significant spread of caries from 70 to 94,6% in the different age groups. The proportion of people with malocclusion among the surveyed reaches 46,6%.

Diagnostics and complex treatment of pain dysfunction syndrome of temporomandibular joint

Summary:

Research objective is to increase quality of diagnostics and treatment of patients with a pain dysfunction syndrome of temporomandibular joint by the choice of diagnostic and medical actions. Material and Methods: 172 patients with a syndrome of pain dysfunction of temporomandibular joint have been underthe survey. All patients have been examined according to the scheme including subjective, objective and express methods. Results of the research allowed to reveal that at 30% of patients the syndrome of pain dysfunction of temporomandibular joint has been caused by disk dislocation. Upon termination of the treatment which has been carried out by the scheme, the normalization of relationship of joint elements has been observed. Conclusion: When the diagnosis of syndrome of pain dysfunction of temporomandibular joint is proved, it is necessary to apply complex treatment.

Oral surgical treatment by erbium laser application in patients with the risk of bleeding

Summary:

Aim: to analyze the erbium laser effects in oral surgery in patients with the risk of bleeding. There were selected 2 groups of patients for oral surgery treatment. Materials and methods. The first group included 31 patients with bleeding risk. In this group erbium laser radiation was used for the cut and tooth socket conditioning after tooth extraction. The second (control) group of 43 patients without concomitant pathology was determined for conventional surgical treatment. Results. In the first group there was no postoperative bleeding, post-surgical pain and infection were prevented with no need for analgesics, and the wounds epithelization took 1-3 days. It takes less time than in control group. Conclusion. Application of erbium laser is a modern method which can be successfully used in surgical treatment of patients with the risk of bleeding.

Surface modification of orthodontic implants by nanocomposite coatings based on chitosan and metals

Summary:

Purpose: to study the properties of nanostructured coatings in orthodontic implants. Material and methods. Low and average molecular mass chitosan, 3-amynopropil-3-methoxysylan and suspensions of 5 nm and 10 nm gold nano-spheres stabilized with natrium citrate were used for nanocomposed surfaces. 2mg/ml polyethylenamin water solution was used for making the underlayer before putting polyion coverage using «POLYION-1M». The polyion covering dynamics was studied by polyquartz weighing method. Morphology of created layers was studied with atomic microscopy, elements were studied by secondary ion mass-spectrometry. Results. The best transmission among structures of chito-san/metals was showed by low molecular mass of chitosan and 8-1 Onm nanoparticles of metals. Analysis of roughness of surface shows that nanoparticles of gold make the most solid surface on 3-amynopropil-3-methoxysylan underlayer. Conclusion. The development of biocomparative materials in maxillofacial surgery is considered to be effective method of decreasing the risk of post-operative inflammatory complications by local antibacterial effect.

Summary:

Objective: To assess the importance of using the new system Quick Sleeper for local anesthesia, to highlight benefits of quick and comfortable anesthesia. Material and Methods. The examination of effectiveness, convenience of this kind of anesthesia has been carried out. Results. All patients, taking part in this examination, confirmed more comfortable condition after this anesthesia than conductor and infiltration methods of anesthesia. The effect of anesthesia is better than after conductor anesthesia. Conclusion. This technology guarantees equal introduction and spread of anesthetic, independently of tissue density, eliminating the risk of carpule breakage.

Summary:

The purpose of the work is to prescribe combined treatment in order to determine the efficiency of cycloferon liniment. Materials and methods. 80 patients suffering from such inflammatory disease of parodentium as herpetic stomatitis and periodontitis have been examined and treated. Results. Cycloferon liniment used in the combined treatment of stomatitis and periodontitis reduces the infection load in gingival pockets, decreases local inflammation, normalizes immunity parameters, decreases endotoxemia and reduces relapse rates. Conclusion. Cycloferon liniment is an efficient medication restoring the parameters of local nonspecific immune response.

Results of practical application of questionnaire to determine the level of dentophobia and dynamics in the doctor— patient relationship

Summary:

Objective: to develop a questionnaire for assessing the level of dentophobia and dynamics of the relationship in the «doctor-patient» in the course of treatment and to conduct its clinical trials. Material and Methods. 90 patients aged 26-56 years with chronic generalized periodontitis have been examined. The research has been conducted by Hospital Anxiety and depression (HADS) questionnaire to determine the level of dentophobia; psychophysiological testing was carried out on the hardware-software complex (APC) «NS-Psychotest» (LLC «Neurosoft», Russia, Ivanovo, 2007). Clinical examination included examination of the mouth and the index of assessment of hard tissue of teeth and peri-odontal tissue: a defining simplified oral hygiene index (UIG), sulcus bleeding index (IR), gingival index (PMA). The survey was carried out before the treatment and 3 months after the treatment. Statistical analysis was performed in the program Statistica. Using non-parametric methods (Wilcoxon test, criterion A2, Spearman correlation coefficient). Statistically significant differences with p < 0,05 were considered. Results. The development of the questionnaire was carried out in accordance with all generally accepted standards. The initial testing was performed easily by patients. During the clinical trials visible results of the proposed questionnaire were obtained that proved its sensitivity to the changes in dental and psychological status of the oral cavity. Conclusion. The practical application of the questionnaire determines the level of dentophobia during the first visit. The data of the questionnaire identify the differentiated deontological approach. It improves the relationship with the patient, reduces his level of dentophobia. The reduced dentophobia promotes compliance and psychologically comfortable communication in the doctor-patient system, which also contributes to the prevention of emotional burn out among dentists.

Summary:

Objective: to study the adhesion of oral microflora and the degree of penetration into the structure of the basis of structural polymers in a comparative perspective. Material and methods. The object of the study is based on samples representative of the modern basis of polymers subjected to contamination by microorganisms in the experiment. Assessment of the quality of the resulting surface samples of basic dental polymer after polishing using a scanning ion microscope. The study of adhesion of microorganisms on the surface of the samples was performed in the experiment in vitro by the method of V. N. Zarev, 2006. Fluorescence imaging was held to study microbial penetration into the interior of polymers of microorganisms using a confocal laser scanning microscope. Results of the study revealed a degree of adhesion and penetration of microorganisms to the surface and the structure of the base polymer. Conclusion. The findings suggest the importance of final basic polymers. Thermoplastic polymers are characterized by complex processing, concerning promote greater adhesion and infiltration of microorganisms into the interior of the construction in comparison with the acrylic polymers.

Psychophysiological support of orthopedic treatment in patients with dento-alveolar form of dentition and occlusion

Summary:

The purpose: to improve the treatment of patients with orthopedic dento-alveolar deformation of dentition and occlusion by using a range of methods of psychophysiological support of patient care. Material and methods. The study included 86 patients, men and women aged from 18 to 49 years with dento-alveolar deformation of dentition and occlusion. The study used clinical, anthropometric, radiological, photometric and static methods. At each stage of treatment, to investigate the characteristics of the functional status of patients methods of SAN, IPC «adaptive», self-esteem of patients by developed questionnaire were used. Results. A direct, reliable, moderate correlation has been established between the estimates that have given quality of care to patients with a regularity of the complex of psychophysiological methods proposed. Conclusion. A positive impact of the proposed complex of psychophysiological methods to support the functional characteristics of the patients and their assessment of the treatment, and the effectiveness of orthopedic treatment have been determined.
