Peculiarities of cardiac hemodynamics and functional state of left ventricular myocardium in teenagers with connective heart tissue dysplasia

The aim of investigation is to study heart hemodynamics in teenagers with connective tissue dysplasia of heart (CTDH). 35 patients ages 12 to 15 years with CTDH have been observed: Group I (n=14) are the patients with mitral valve prolapse (MVP) without mitral regurgitation (MR) and myxomatous degeneration(MD) or isolated minor heart abnormalities (MHA), Group II (n=11)are patients with MVP and MR in combination with 1 or2 MHA, and Group III (n=10) are patients with MVP and mixoid degeneration (MD) in combination with 2 or more MHA. The control group consisted of 15 patients of the same age without MHA. All the children passed Doppler and echocardiography. According to the results significant changes of cardiohemodynamic indices in patients of Group I were not observed. The changes of size and volume indices of the left ventricle (LV), increase in wall thickness and diastolic dysfunction of the LV were observed in Group II. The significant changes of systolic function of left ventricular myocardium were observed in Group III. The study comes to the conclusion that teenagers with CTDH have definite changes of heart hemodynamics and functional state of left ventricular myocardium. These changes depend on mitral regurgitation, myxomatous degeneration and MHA combination
Retrospective analysis of follow-up results in patients with skin lymphomas of low degree malignancy

Administration of specific chemotherapy (cytostatics) has great significance in the treatment of skin lymphomas of low degree malignancy. The research goal is to study follow-up results of cytostatic therapy of skin lymphomas. Retrospective observation of survival of patients with T-cell epidermothropic skin lymphomas using special therapy and without its use has been studied. Comparative analysis of survival rate in two groups of patients has been done. 40 patients received cytostatics and 32 patents were not treated by chemotherapy. The first group of patients showed the reduction of survival level and increase of mortality level from skin lymphomas of low degree malignancy. The research findings proved the influence of cytostatic therapy on the survival of patients with skin lymphomas of low degree malignancy

Hypophysis adenomas are mostly spread in the chiasmatic cellular area. They account 18% of all new brain formations, the structure of pituitary adenomas includes prolactinomas in a large number of cases which are manifested by the syndrome of hyperprolactinemia and hormone inactive hypophysis tumours (35%). Somatotropins (13-15%) are lower in frequency, the main clinical feature is acromegalia. One can rarely reveal corticotropins (8-10%), gonadotro-pins (7-9%) and thyrotropins (1%) and their mixed forms. Transsphenoidal surgical interventions are considered to be methods of choice treatment of hypophysis adenomas and other formations in the chiasmatic cellular area. Alternative methods of treatment are conservative. They can be as an addition to microsurgery (radiotherapy)

The high cardiovascular mortality (CVM) proves the necessity of verification analysis of cardiovascular death causes. The research goal is to determine the quality, facilities and peculiarities of cardiovascular diseases (CVD) of postmortem diagnostics and statistical recording as a cause of death. 1972 successive cases of CVM in Voronezh, Ryazan and Khanty-Mansiysk have been analyzed. The majority of deaths caused by CVD were classified as fatalities occurring outside hospital and in the absence of medical care (88.0%). Incidence of cardiovascular deaths was confirmed by autopsy in 28.3% cases. With increasing age, the incidence of detecting CVD as a cause of death increased, while the frequency of referral to autopsy consistently decreased. Chronic Ischemic Heart Disease (IHD) forms comprising 48.3% of all CVM were confirmed by autopsy results only in 29.5% of cases. Acute IHD forms (including myocardial infarction) accounted for only 11.6% of CVM; however this diagnosis was based on autopsy results in 72.4% of cases. The high death rate outside hospital and in the absence of medical care with the rare referral to autopsy, especially for elderly having chronic CVD, does not provide reliable statistical information on CVM
