Migreine and periictal headache in patients with epilepsy

Migraine and epilepsy are diseases characterized by paroxysmal occurring clinical manifestations that have different mechanisms of pathogenesis. In the interictal period in 59% of patients with epilepsy there are different types of primary headache: 18%, migraine without aura, 33% from tension-type headache, and 8% not classified headache. We have analyzed clinical and electrophysiological pattern in patients with migraine and idiopathic generalized epilepsy. A preliminary analysis of the clinical picture of headaches in adult patients suffering from idiopathic generalized epilepsy suggests that migraine occurs more often in patients with idiopathic generalized epilepsy, whereas among patients with focal symptomatic epilepsy is more common patients with secondary headache
The role of transient ischemic attacks in the occurrence of cognitive impairment

There were considered primary risk factors and pathogenesis of the occurrence of TIA and cognitive impairment. The algorithm of examination of patients with TIA was presented.
Headache features in persons with anxiety disorders

The aim of the article is to estimate the impact of anxiety on personality characteristics of headache in young adults studying full-time. Material and Methods. We observed 92 people aged from 15 to 25 years old, socially adapted, students in schools, colleges and universities on a full-time offices. Results. The perception of headache strongly depends on the emotional state of the individual, so it is very important to take this into account in the analysis of complaints and anamnesis of patients. Conclusion. Anxiety personality disorders hinder objectifying cephalgia, leading to misdiagnosis and incorrect treatment tactically with the possible development of abuse headaches. Therefore, it is necessary to convey to the patient the importance of reducing the impact of modifiable risk factors to explain the relationship of his condition with these factors and create a positive attitude to treatment.
Cognitive impairment in multiple sclerosis

Aim: to identify the degree of cognitive impairment (CN) and to optimize the treatment of patients with multiple sclerosis (MS). Material and methods. A total of 695 patients (278 men and 417 women) were ranged from 18 to 63 years. The mean age was 30.2±0.7 years: women (417) 28.5±0.5 years, while for men (278) 31.8±0.7 years. Relaps-ing-remitting type (RT) of MS was established in 520 patients (74.8%), secondary progressive type (VPT MS) in 132 patients (18.9%) and primary progressive type (PPT MS) in 10 patients (1.5%). Clinically isolated syndrome (CIS) was detected in 33 patients (4.8%). The diagnosis of MS 662 patients according to the criteria McDonald etal. (2005). Score of neurologic deficit was carried out on an extended scale of disability (Expanded Disability Status Scale — EDSS). CN were evaluated by conventional tests. To estimate the orientation in time, assessment of short-term and long-term memory, attention and concentration, as well as executive functions, memory, language, evaluation of optical-spatial activities, conceptual thinking, the account used by the Montreal Cognitive Assessment Scale (MoCA). For the screening of dementia with a primary lesion of the frontal lobes and subcortical cerebral structures used battery frontal test to assess frontal dysfunction. Results. The ratio of male (265) and female (397) was 1:1.5. The severity of the condition patients EDSS scale ranged from 1.5 to 8.0 points, and the average score was 3.5±1.2. In the group of patients with RT RS average score EDSS was more than a half (2.5±1.1), than in the group of patients with MS VAC (5.5±1.2) and POS PC (6.5±1.2). In the study of history, it was found that the development of the RS (662 patients) was preceded by the following conditions: a viral infection in 277 patients (41.84%); fatigue in 147 patients (22.21%); transferred psycho-emotional load from 218 (32.93%); after pregnancy and childbirth in 20 patients (3.02%). Conclusion. Among the patients with MS often met type remitting course of the disease. CN were diagnosed in 82.93% of patients. Some patients KN come to the fore in the onset of the disease, with minimal neurologic deficit, which significantly complicates the treatment and impairs the subsequent social adaptation of this group of patients. In the patients with a VPT MS KH level was statistically significantly lower than in patients with RT RS.

In order to predict and detect neurological complications at somatic diseases earlier, there have recently been started to use some of the markers determined in the patient's blood serum. Aim of the study. The aim of the work was to evaluate the activity of neuron specific enolase (NSE) in patients with primary hypothyroidism (PHT). Materials and methods. In 110 patients with PHT detailed study of neurological and neuropsychological status, determined NSE level in serum. Results. The results of this study showed that the NSE activity depended on the gender, age of the patient, the duration of existence of the endocrinopathy and the presence / absence of neurological symptoms. Conclusion. Produced a comprehensive assessment of the state of the nervous system in patients with PHT, NSE defined value as a non-specific marker NA lesions in these patients. The goal set at the beginning of the study, achieved.
Epidemiological trends of neurosyphilis in Saratov region

The aim of the article is studying the epidemiological trends of neurosyphilis in Saratov region between 1993 and 2014. Material and methods. The forms of state statistical inquiry in Saratov region have been studied. Results. The incidence of syphilis and increase the incidence of neurosyphilis in Saratov region was confirmed. Late manifest forms were predominant. Conclusion. Neurosyphilis is not a rare disease of the nervous system in Saratov region in the past years. Closer cooperation between neurologists and dermatologists is needed.
The use of low intensity laser radiation in the treatment of patients with closed traumatic brain injury consequences

Objective: to determine the effectiveness of the use of low-intensity helium-neon laser radiation in patients with post-traumatic cerebral arachnoiditis. Material and Methods. We observed 164 patients with the effects of closed craniocerebral injuries of a various severity. Methods of percutaneous laser irradiation of blood in the projection of the common carotid artery at the level of IV cervical vertebra was carried out underthe supervision of the clinical evaluation of hemodynamic data survey methods. Results. Positive results of hemodynamic parameters of examination methods indicate regression of cerebral vasospasm and improving brain vascular reserve, which led to a decrease in cerebral and focal neurological symptoms. Conclusion. The method allows the use of this technique in these patients both in the hospital and at the outpatient stage.
Remote results of observation patients with cerebral palsy in spastic diplegia form after course of muscles electrostimulation

The aim of the research is to estimate remote results of observation patients with cerebral palsy in spastic diplegia form. Materials and methods. Under observation were 71 patients with cerebral palsy in spastic diplegia form in the age from 3 to 16 years were described in the present article. Parameters of walking were registrated before, after treatment and 6 months later have finished of treatment. Patients were distributed by two groups. 38 children were included into the first group. They have gotten course of complex rehabilitation with addition functional programmed muscles electrostimulation. The second group was composed by 33 patients. These children have been spent a course of complex rehabilitation like first group besides type of electrostimulation. Second group of patients received passive electrostimulation. Results. At the children of the first group a reliably (p<0,05) good remote result (increasing indexes to 35%) had been registrated in comparison with them condition after treatment. The first group of patients had reliably (p<0,05) good result then the second group. Conclusion. Significantly good results on several symptoms (limping, moving in knee and coxal joints) received with use functional programmed electrostimulation of muscles in children with spastic diplegia form of cerebral palsy.
