Clinicodiagnostic value of functional morphological changes of intestine in encephalopathy development in cirrhosis

Summary:

The aim of the article is to study frequency and expressiveness of clinico-morphological and dysbiotic intestinal changes in cirrhosis and to estimate its interrelation with manifestations of hepatic encephalopathy. 146 patients with cirrhosis ages 20 to 70 years have been examined. The cirrhosis has been verified by standard methods. Examina¬tion included colonoscopy and stool specimen. Diagnostics of hepatic encephalopathy was based on clinical and psychometric methods. It is established that symptoms of intestinal dyspepsia often occurred in patients with cirrhosis of classes В and С by Chaild-Pugh. According to the endoscopic examination lesions of mucous membrane of large intestine have been revealed in 82% of patients with liver cirrhosis. Morphological research revealed changes in 100% of patients with cirrhosis. Frequency and expressiveness of changes in mucous membrane of large intestine depended on the degree of cirrhosis decompensation. Changes of biocenosis of large intestine were revealed in 93% of patients with cirrhosis, hepatic encephalopathy was diagnosed in 88% of patients, more frequently hepatic encephalopathy of stage I. The cirrhosis is accompanied by structural and functional changes of intestine that make up additional risk factor of development of hepatic encephalopathy

Summary:

100 patients with inflammatory diseases of gastrointestinal tract were examined. Microbial component of inflammation process was detected by means of polymerase chain reaction (PCR). By means of PCR etiological microbial factor of mononuclear cells in peripheral blood (MCPB) was investigated. Mixtinfication of mucosa membrane of gastrointestinal tract with H. pylori and Herpesviridae in pathology of gastroduodenal area and presence of fragments of microorganisms DNA in PBMC were revealed. This phenomenon may be important in pathogenesis of pathology of gastrointestinal tract and forms the basis for its further investigation

Occupational risk of HIV infection among dental surgeons in Nigerian

Summary:

Background: Prevention of accidents and management of exposures in the work environment is an important oc¬cupation health issue. This study objective was to investigate the occupational risk of HIV among Nigerian dental sur¬geons. Methods: This descriptive cross sectional survey of 300 dental surgeons practicing in private and government owned dental centers in Nigeria was conducted from June 2006 to January 2007. Results: Percutaneous injury was recorded among 69.3% of respondents and only 1.2% had Post exposure prophylaxis. Those with abraded skin that will treat patient without additional barrier were 8.6%. Percutaneous injury was positively related to gender, position, additional qualifications (p<0.05). Conclusion: Percutaneous injury is significantly high and low preventive measure at such exposure. Policies, practices, and trainings geared towards protecting and reducing the prevalence of percu¬taneous injury among dental surgeons, and improving post exposure prophylaxis uptake in the event of exposure is a necessity

Tegument based in-silico Drug Targeting of Herpes Simplex Virus-1

Summary:

Alpha Trans Inducing Factor (a-TIF) is a herpes simplex virus type 1 (HSV-1) virion tegument protein present in the tegument layer between the capsid and the envelope, in association with cellular proteins and trans-activated viral activities. a-TIF stimulates the transcription of HSV-1 Immediate Early genes during lytic virus replication. The presence or absence of functional a-TIF protein in vivo is always associated with viral activities. Its role behind latent HSV-1 infection is ambiguous. No drug is designed for HSV till now based on this protein and thus its conformational details can be very important for drug designing purpose. Methods. Docking studies on this protein becomes logical. Four different ligand molecules viz. Adenosine-3'-5'-Diphosphate, P1-(5'-Adenosyl) P4-(5'-(2'-Deoxy-Thymidyl)) Tet-raphosphate, 9-Hydroxymethylguanine, 9-(4-Hydroxybutyl)-N2-Phenylguanine were screened for the study. Results. P1-(5'-Adenosyl) P4-(5'-(2'-Deoxy-Thymidyl) Tetraphophate was found to be the best ligand as it showed the lowest docking energy of - 9.238 kcal/mol. Conclusions. The best ligand was found to bind different sites of the a-TIF protein and hence can be utilized to combat HSV-1 successfully, with a synergistic effect of multiplicity of drug molecules on the target protein

The treatment of nystagmus by periodic light influence

Summary:

The data about results of treatment of nystagmus by periodic light influence are brought. 56 patients with various kinds of nystagmus received the treatment. After treatment the stable reduction of nystagmus' amplitude was observed in 85 % of cases. The efficiency of treatment raised at reduction of a time interval between repeated courses of treatment. It is marked, that at an arrangement of light sources along the horizontal axis parallel to an axis of eyes' arrangement, at vertical and rotator nystagmus light influence can suppress, for example, a horizontal component of nystagmus, poorly influencing on its vertical component. It is shown, that accommodation of light sources in these cases in a frontal plane with an opportunity of a choice of it's position can raise efficiency of treatment of vertical or rotator nystagmus

Dynamic thermal imaging control of an eye in postoperative period after cataract phacoemulsification

Summary:

The research goal is to study the impact of various schemes of antibacterial therapy in the postoperative period in patients after cataract phacoemulsification with distant thermography. Under observation there were 68 patients with cortical initial cataract. All patients were subjected to cataract phacoemulsification. Patients were divided into 2 groups: 34 patients of the first group after the operation dropped into the operated eye medication «Oftaquiks» 1 drop 5 times a day for one day before surgery and for 5 days postoperatively. 34 patients from the second group as an antibacterial drug received medication "Cipromed" in a similar way. Inflammatory reaction in postoperative period in the first group was not evident. In the second group the presence of inflammatory reaction of various degrees was revealed. Period of its reduction was 6,3 ± 2 days. The temperature difference between the non-operated and operated eye in the 3d day in most cases was less than 1 day after surgery. In the first group of patients the average temperature difference in the 3d day after surgery was 0.35 ± 0.21 ° C, and in the second group of patients - 1.3±0.83°C. Advantages of dynamic thermal imaging method for monitoring the state of the eye, including the analysis of time dependencies of temperature in the process of natural load test, which allows to select the optimal drug correction of postoperative cataract pha-coemulsification were revealed

The dynamic of aterosclerosis affection on brachio-cefalic arterys of the high risk patients under the hypolipidemic treatment

Summary:

Purpose. To study the statin influence on lipid exchange and the degree of the brachiocefalic arterys alteration. Methods. The research included 33 high risk patients. The patients inverstigation included lipid spectrum, blood coagulation indexes, by the means of triplex scanning of the brachiochefalic arterys the thickness of the intima - media complex and the expression of stenosis of carotid, vertebral and subclavian arterys were measured. Results. All the patients were divided into the groups according to the total cholesterol and low density lipoprotein cholesterol levels. Patients were divided into subgroups according to the expression of stenosis. The 1 group of patients received rosuvastating 10 mg, the 2 group - atorvastain 20 mg, 3the 3 group - simvastatin 40 mg. This inverstigation shows that the high risk patients with high levels of total cholesterol and low density lipoproteins have the highest expressiveness of brachiocefalic arterys atherosclerosis affection. Conclusion. The assignment of the statins to these patients is accompanied by the lowering of the levels of total cholesterol and low density lipoproteins and the quantity of arteries affected by the atherosclerosis and also by decreasing of the stenosis expression

Significance of bioregulation and indications to its usage in complex treatment of thyrotoxic myocardiodystrophy

Summary:

Significance of biological feedback (BFB) therapy in complex treatment of patients with thyrotoxic myocardiodystrophy was analyzed. Evaluation of bioregulation procedures was conducted in 36 patients with diffusive toxic goiter (DTG) and signs of thyrotoxic myocardiodystrophy on the background of drug therapy. Control group consisted of 24 patients with DTG, which took only drug therapy. It was revealed, that combination of drug therapy of DTG with BFB procedures provided significant influence on state of vegetative nervous system, and it also led to quick decrease of clinical symptoms of thyrotoxic myocardiodystrophy, in contrast to isolated drug therapy

Evaluation of five-year risk of lethal outcome and development of cardiovascular disorders in patients with acute myocardial infarction on basis of 0.1-Hz rhythms synchronization in cardiovascular system

Summary:

The research goal is to evaluate the impact of autonomic heart control indices on five-year risk of lethal outcome and development of cardiovascular disorders in patients with acute myocardial infarction (AMI) and to study the dynam¬ics of indices in short- and long-term period. 125 patients with AMI (42% female), ages 30 to 83 years, were enrolled in prospective observational study. Observation period lasted 6 years. Control checking was carried out in 1 week, 3 weeks, 6 months, 1 year and then annually. The dynamics of heart rate variability (HRV) and the synchronization of 0.1-Hz rhythms of heart and microcirculation (S) were studied during the first year. Outcomes proved to be death, MI, stroke. Development of acute heart failure Killip 2-4 and indices S < 20% during the first week of AMI (х2 = 10,5, p = 0,005 for the Cox model) played a significant role in evaluation of five-year risk of death after AMI. Sensitivity (Se) and specificity (Sp) of index S < 20% during the first week of AMI were 76% and 43% correspondingly. The Cox model showed that indices of HRV and 0.1-Hz rhythms synchronization were not of great value in evaluation of five-year risk of death after AMI. The article concludes that indices S < 20% in patients with AMI possess better prognostic value than common clinical parameters
