Analysis of structure of late posthospitalized chronic recurrent purulent wound complications after operations on organs of abdomen and retroperitoneum

Summary:

Materials for this research were the results of complex examination and surgical treatment of 395 patients with chronic recurrent posthospitalized purulent wound complications re-admitted to the department of purulent surgery of City Clinical Hospital №2 n.a. VI. Razumovsky over five-year period; for the period from 2 months to 38 years after the primary undergone surgery on organs of abdominal and retroperitoneal space. On the basis of clinical data, surgical and biopsy material and operating discoveries the frequency and nature of postoperative complications of wound process during the remote posthospitalized period were studied. Structure of recurrent inflammatory purulent processes represented 4 groups of complications: I - «ligature» complications - 107 (27,1%); II - nonligature genesis complications- 199 (50,4%); III -complications that are not related to the transferred operations-25 (6,3%); IV-complications due to a combination of reasons - 64 (16,2%). Clinical pathological processes in each group of complications were determined in the form of abscesses, fistulas, infiltrates in the projection of postoperative scar and classified in the case history as «ligature». By etiology, pathogenesis and morphology they presented a variety of nosological forms. Each clinical and morphological form had its own nuances of the clinical course, treatment and prognosis, which were not occurred during routine diagnostic methods. They occurred to be visible during retrospective analysis and had practical significance in the correct choice of surgical treatment

Summary:

The aim of this study was the estimation of clinical importance markers of renal damage at surgical treatment of nephrolithiasis. We studied results of operations in 110 patients with nephrolithiasis. The study presents new candidates for biochemical markers of acute kidney injury and renal interstitial fibrosis, with potentially high sensitivity and specificity. In this study were measured the concentrations of these biomarkers in urine. We suggested, that increased concentrations molecular mediators of renal damage (L2, IL-18) and interstitial fibrosis (MCP-1, TGFp, IGF1) in the patients with nephrophibrosis and strongetubulointerstitial injury, saved long-time period after operation, has been shown of progression chronic kidney disease

Actual aspects of chronic cystitis pharmacotherapy in women in exacerbation period

Summary:

60 women were examined with the diagnosis of chronic cystitis in exacerbation period. Cystoscopy with biopsy of urinary bladder mucous was made on the first day and the tenth day. The patients were examined by means of the battery of psychometric and psychological methods. Women were divided into two equal groups. The first group took Furamag, the second group - Furamag and Adaptol. The patients complained of dysuria on the first day There was moderate leukocyturia in the urinalysis. Intensive leukocytic infiltration was determined in the biopsy of urinary bladder mucous. In assessing psychic status in patients, the tendency to social isolation was determined in both groups for the first day Laboratory findings were normalized on the tenth day; the dysuria was cropped in both groups in 100 %. Leukocytic infiltration in the biopsy of urinary bladder mucous was changed by lymphocytic one, and proliferative processes were increased. In the second group of patients more rapid relief of anxiety and depressive disorders was noted

Summary:

80 cytological preparations derived by fibrogastroduodenoscopy from 20 patients with cerebrospinal trauma at cervical part level served as the research subject. Dynamics of the progression of acute erosive ulcered lesions of mucous membrane of the upper parts of the gastrointestinal tract was studied on the basis of the cytological analyses of mucous membrane biopsy materials. In the course of our work we used endoscopic (fibrogastroduodenoscopy) and cytological research methods. Cytological analyses of mucous membrane biopsy materials were carried out on the 7th, 14th, 21st and 28th day. Biopsy material cellular composition was evaluated on the grounds of the calculation of neutrophilic leukocytes and epithelial cells with the use of an immersion objective. In so doing we registered neutrophilic leukocyte number for 100 cells and determined neutrophilic and epithelial index. Monitoring of neutrophilic leukocyte number enables to determine presence or absence of inflammatory changes in stomach mucous membrane and duodenum in different periods of cerebrospinal trauma

Influence of gravitational therapy on reparative osteogenesis in patients with osteomyelitis of lower extremities

Summary:

The article provides a comparative analysis of reparative osteogenesis in 105 patients with diffuse osteomyelitis of lower extremities, covering bone in diameter totally or subtotally. In complex treatment 51 patients have experienced gravitational therapy (study group), 54 patients have been treated by standard methods (control group). The study was conducted in clinics of Samara State Medical University. Gravitational therapy is a new noninvasive method of physiotherapy, first used in the treatment of patients with osteomyelitis. It is noted that the inclusion of gravitational therapy in complex treatment contributes to the consolidation of bone fragments, accelerates о ste о reparative processes in the regenerate bone formed in the zone of the defect in osteosynthesis in apparatus for external fixation. Past radionuclide study of the skeleton showed that the gravitational therapy positively affects the blood supply to the area osteomyelitic defeat, activates and improves bone metabolism. Osteodensimetric studies have shown that under the influence of gravitational therapy there was an increase in bone mineral density of the lower extremities, especially with the affected side, which was not observed in patients of control group. Gravitational therapy is an effective method of influence on reparative osteogenesis in patients with osteomyelitis of lower extremities
