Иммунопатогенетические механизмы кожных токсических реакций противоопухолевой терапии мультикиназными ингибиторами ангиогенеза

Резюме:

 Нарушение ангиогенеза связано с патогенезом многих заболеваний, но особенно выраженный патологический ангиогенез лежит в основе роста опухолей и метастазирования. Физиологический контроль над процессами ангиогенеза осуществляется многими факторами роста, один из основных — вазоэндотелиальный фактор роста VEGF, реализующий свои эффекты через рецепторы — протеинкиназы. Таргетная терапия различных опухолевых заболеваний, направленная на подавление ангиогенеза, нацелена на блокаду рецепторов к VEGF и другим проангиогенным факторам роста, но лекарственная блокада рецепторов к факторам роста и подавление опухолевого ангиогенеза ведут к нецелевому воздействию на основные клетки дермы — фибробласты. В результате изменения регуляторных механизмов, торможения физиологического обновления эпидермиса, подавления ангиогенеза и репарации при повреждении кожи появляются различные кожные токсические реакции, зависящие от рецепторов-мишеней ингибиторов ангиогенеза. Изучение механизмов нежелательных явлений таргетной терапии является актуальным направлением онкоиммунологии и дерматологии, что в дальнейшем поможет определить оптимальные схемы коррекции кожных токсических реакций и добиться максимальной эффективности терапии опухолей.
