Summary:

Determination of serum lipidic spectrum (increase of total cholesterol, atherogenic rate) for dynamic control of possible transformation in cholelithiasis is necessary to be included in examination of patients with cholecystitis. Changes in serum lipidic level (increase of total cholesterol, rate of total cholesterol to cholesterol of lipoproteins of high density and decrease of lipoproteins of high density) in healthy people promote forming risk group of gall-bladder cholestherosis

Pharmacoeconomic efficacy of complex medical and climatic treatment of atopic asthma in Teberda resort

Summary:

120 patients with atopic asthma have been divided into two groups: the control group (60 patients) - has been treated with budesonide and formoterol combination (Cymbicort Turbuhaler) in individual doses; the main group (60 patients) additionally has being taken a course of climatic therapy in Teberda resort for 21 days. Common pharmacoeconomic analysis has been carried out with study of «expenses-efficiency» balance. Statistic results have been processed with Statistica 6,0 program. Complex of medical and climatic treatment of atopic asthma in Teberda resort promoted twice reduction of Cymbicort Turbuhaler dosage in 63,33% of patients while holding asthma control. Main group patients significantly rarely asked for stationary, out-patient or emergency aid; so it helped to reduce yearly expenses for 1 patient treatment to 51, 69%. Complex medical and climatic treatment of atopic asthma in Teberda resort allows to reduce pharmacoeconomic expenses significantly and to improve disease course

Heart rate variability, structural and functional characteristics of the left ventricle in essential arterial hypertension

Summary:

For the purpose of diagnostic and treatment improvement it is necessary to investigate relationship between different pathogenetic mechanisms in essential hypertension. The aim of our study was the comparative analysis of the parameters of cardiac remodeling and neurohumoral mechanisms of heart rate variability (HRV) in patients with essential arterial hypertension. 100 patients with essential arterial hypertension were included in the study. All patients went through heart rate variability parameters and left ventricle characteristics evaluation. Time and frequency domains were decreased in patients with arterial hypertension. Excess sympathetic activation takes place, parasympathetic system is depressed and it shows pathogenetic basis of disadaptation. Left ventricle hypertrophy is associated with progressive decrease of HRV and its circadian fluctuations. Concentric left ventricle hypertrophy is characterized by the most significant decrease in time and frequency domains of HRV. 1/3 of hypertensive patients with normal left ventricle geometry have diastolic dysfunction and also some decrease in time and frequency domains. In conclusion, we found that cardiac remodeling in patients with essential hypertension seems to be related to the severity of impairment of cardiac autonomic control

The article presents the research results on the correlation between anemic syndrome and main risk factors in hospitalized patients with different forms of cardiovascular pathologies. This category of patients is characterized by more severe arterial hypertension and low level of total cholesterol in blood. It has been revealed that anemia in patients with CVD (cardiovascular diseases) is not only associated with unfavorable prognosis, but also becomes a marker of peculiarities in mechanism of disease development indicating the severity of inflammatory component

Peculiarities of dyslipidemia in patients with psoriatic arthritis: connection with atherosclerosis, cardiovascular risk factors and inflammation activity

Summary:

Objective: to found the dyslipidemia in patients with psoriatic arthritis and to study the connection between dyslipidemia and cardiovascular risk factors, atherosclerosis and inflammation activity. 40 persons with PsA without cardiovascular diseases were involved in the study, 25 healthy people were examined like controls. Activity of PsA was learned by DAS, Likert index, Ritchie Arthicular Index, Number of swelling joints (NSJ), ESR, C-reactive protein (CRP) and fibrinogen. Total cholesterol, triglycerides, low and high density lipoprotein cholesterol, hypertension, body mass index, individual cardiac history were performed like cardiovascular risk markers. The ultrasound measuring the thickness of intima-media layer (IML) in carotid arteries was performed to subclinical atherosclerosis study. Increase of total cholesterol, triglycerides and low density lipoprotein cholesterol level was found in patients with PsA comparative with controls. There was prevalence of high and moderate increase of total cholesterol in patients with PsA, and in controls only low increase was measured. Correlation between total cholesterol and NSJ, fibrinogen, hypertension and IML was found. Low density lipoproteins were tingly interrelated with ESR, hypertension and IML. Very low density lipoproteins were connected with age of disease beginning, hypertension and IML, and triglycerides-with hypertension, enthesitis and dactilitis. Dyslipidemia in patients with PsA characterizes by total cholesterol, triglycerides, low density lipoprotein cholesterol increase, but not high density lipoprotein decrease. There is the connection between dyslipidemia in PsA and inflammation activity, arterial hypertension and IML

CAT-test significance in life quality assessment in patients with chronic obstructive pulmonary disease

Summary:

Comprehensive analysis of the impact of complex vaccination against pneumococcus, Haemophilus influenzae type b and influenza on performance CAT-test 6 months after vaccination in patients with chronic obstructive pulmonary disease compared with unvaccinated patients. 1 group - 48 patients with chronic obstructive pulmonary disease, vaccinated simultaneously «Pneumo-23», «Hiberix», «Grippol plus» (mean age - 61,46±1,17 years). Group 2-80 non-vaccinated patients with chronic obstructive pulmonary disease (mean age - 54,65±0,6 years). Patients from both groups held CAT-test at baseline and 6 months after. There was a significant decline in CAT-test after 6 months in the vaccinated patients compared with unvaccinated patients with chronic obstructive pulmonary disease.
CAT-test can be used as a sensitive tool to monitor indicators of quality of life in patients with chronic obstructive pulmonary disease in the process of therapy. Complete vaccination against pneumococcus, Haemophilus influenzae type b infection and influenza contributes to improving the quality of life in patients with chronic obstructive pulmonary disease in 6 months after medical intervention

Summary:

Aims: To study the aspects of cardiac neuropathy (CN) early formation in patients with metabolic syndrome (MS) and carbohydrate metabolism malfunction according to the data of cardiac rhythm variability analysis (CRV). Vegetative status assessment in patients with MS and glucose tolerance disturbance (GTD), MS and type 2 diabetes mellitus (DM-2) has been done relying on the investigation of CRV analysis conventional features, such as RMSSD, pNN50, new approach application based on the determination of rhythmogram short period variations (RSPV), and rhythmogram weighted-mean variations (RWMV). RSPV monotony rise failure has been revealed on decrease of the heart contraction rate in patients with MS and GTD, MS and DM-2. Considerable decrease of RWMV has been observed in patients with MS and DM-2. RWMV values determined within 24 hours have been diminished in the morning in patients with MS and DM-2 and normal at night. RMSSD and pNN50 features have appeared to be significantly diminished in patients with MS and DM-2, they ranging within the normal readings in patients with MS and GTD, though in the morning pNN50 values have been found at the lower tolerance. Primary and completely reversible manifestations of CN have been revealed in patients with MS and GTD; new RWMV feature has been noted to have advantages over the conventional characteristics when diagnosing the signs of CN in patients with MS and GTD

Clinical and morphological features of peptic ulcer at men and women

Summary:

The aim of research was to study clinical and morphological features of peptic ulcer at men and women of various age.
166 patients with duodenal peptic ulcer (80 men and 86 women) and 138 with stomach peptic ulcer (80 men and 58 women) were inspected. Clinical displays, morphometrical analysis of components of diffuse neuroendocrine system and stomach receptors of sexual hormones were studied.
It is defined, that peptic ulcer at women under 40 years has more favorable clinical current, than at men. This is based on local neuroendocrine status with positive shifts of melatonin-, VEGF-, glucagon-like cells. In patients aged upwards 40 clinical features of disease course, local neuroendocrine status are erased both at men and women. In patho-genesis of peptic ulcer infringement of receptors expression of steroid hormones in stomach with hyper expression of androgen receptors at men or decrease in receptors expression of estrogen and progesterone along with increase of expression of androgen receptors at women matters.
Forecasting of peptic ulcer can be based not only on traditional criteria, but also on results of dynamic research of androgen receptors expression (at men), androgen-, estrogen and progesterone receptors (at women)
