The choice of performing tactics of dorsal decompressive-stabilizing interventions on the lumbar spine for the prevention of degenerative disease of an adjacent segment

Summary:

Purpose: Comparative analysis of the results of dorsal decompressive-stabilizing interventions (DDSI) on the lumbar spine for substantiating the method of prevention of adjacent segment degenerative disease (ASDd). Material and Methods: 79 patients were included in the retrospective research, 2 groups were identified: in group I (n=41) single-level research, in group II (n=38) two-level DDSI, with rigid stabilization of the spinal motion segment, in the presence of signs of its degeneration was made. Clinical outcomes, instrumental data and surgical complications were evaluated. Results: It was a decrease of pain syndrome in the postoperative period, an improvement in the quality of life in two groups (p<0.001). In the inter-group comparison of the visual analog scale (VAS) — back, legs, ODI, SF-36 in group II, the results at discharge were lower than in group I (p<0.001), after 3.6.12 months there is a comparable improvement in a quality of life and a decrease in pain by VAS in both groups (p>0.05). In the distant period (24, 36 months), clinical results are better in-group II (p<0.05). The number of late surgical complications and revision interventions is higher in a group II (17.1 and 4.0%) (p<0.05). Conclusion: Preventive rigid stabilization of adjacent segment with signs of initial degeneration improves long-term results, reduces the number of repeated operations, despite the larger volume of primary medical intervention.

Clinical and immunological evaluation of pathological changes in the organ of vision in patients with open-angle glaucoma

Summary:

The purpose is to perform clinical assessment of neurodegenerative and inflammatory changes in the visual organ in patients with advanced open-angle glaucoma and to identify the relationship between the degree of neurodegenerative and inflammatory changes. Material and Methods. Prospective examination of 26 patients with advanced open-angle glaucoma and 22 patients without changes in eye hydrodynamics with determination of TGF-p and MMR-9 concentrations in lacrimal and anterior chamber aqueous humor. Results. There was a statistically significant decrease in the concentration of MMR-9 in patients with glaucoma by 1.62 times in the lacrimal fluid (p<0.01) and by 2.17 times in the anterior chamber aqueous humor (p<0.05) in comparison with the control group. There was a statistically significant inverse relationship between MMP-9 in the lacrimal and the indicator characterizing a clinical picture of inflammatory reaction of the conjunctiva (p=0.02) and direct interdependence of the concentration of MMP-9 in the lacrimal and the thickness of the nerve fiber layer (p=0.02). Conclusion. The study revealed correlations between the degree of severity of the conjunctival inflammatory response, neurodegenerative changes, and the content of MMR-9 in the lacrimal and aqueous humor of the anterior chamber of patients with open-angle glaucoma.

Evaluation of the efficiency of simultaneous rigid fixing and interspinous stabilization in patients with degenerative diseases of the lumbar spine

Summary:

Purpose: to assess the effectiveness of simultaneous rigid fixation and interspinous stabilization in patients with degenerative diseases of the lumbar spine. Material and Methods. The study included 216 patients with degenerative diseases of the lumbar spine, who in 109 cases underwent isolated rigid fixation, and in 107 cases, additional simultaneous interspecific stabilization. Results. When compared with the group of isolated rigid fixation, when conducting an additional simultaneous interspinous stabilization of the adjacent segment, lower indices of the level of pain syndrome and better functional status were noted. The total number of postoperative complications in the first group is 17.4%, in the second 5.6% (p=0.02). According to the instrumental data, a statistically significantly lower degeneration of the adjacent segment in group II (p<0.001) was noted, while in 11 group I patients additional decompression-stabilizing interventions at the adjacent level were performed. Conclusion. Simultaneous rigid fixation with the installation of an interspinous stabilizer at an adjacent level in the treatment of patients with degenerative diseases of the lumbar spine can significantly reduce the level of pain, improve functional status and also protect the adjacent segment from the progression of the degenerative process in it and reduce the number of repeated surgical interventions.

Hormonal and lipid imbalances in the formation of myopic chorioretinal neovascularization in women

Summary:

Purpose: to determine the role of hormonal and lipid imbalance in the formation of myopic chorioretinal neovascularization in women. Material and Methods. The frst clinical group includes the results of the study of patients with the active form of myopic chorioretinal neovascularization, the second — the results of the study of their paired eyes. The control groups were formed separately. Ophthalmologic and somatic status of all patients and people in the control groups were thoroughly examined. Results. An increase in the level of prolactin to 501.7±28.2 mlU/ml was established, in the control group to 272.9±33.4 mlU/ml, the level of follicle-stimulating hormone 13±0.1 mlU/ml, the control group 4,6±1.2 mlU/ml. The concentration of estradiol is reduced 35.2±3.1 pg/l, progesterone 2.8±0.1 nmol/l, compared to the control group 66.8±8.1 and 6.2±1.1 nmol/l respectively. An increase in the level of acute-phase proteins was revealed: fibrinogen 3.8 (14.0±0.5) and C-reactive protein 2 times (5.72±2.30) compared with the control group 3.6±0.5 and 2.5±0.7. Increased lipoprotein level (a) 50.2±1.2 g/l and ApoB/ApoA 0.6±0.1 compared with the control group 9.1±1.1 et 0.4±0.1 g/l. Conclusion. Lipid peroxidation and glutathione antiox-idant protection in patients with an exacerbation of chronic pancreatitis

Summary:

Aim: to assess the levels of lipid peroxidation (LPO) products of malondialdehyde (MDA) and glutathione system in patients with the exacerbation of chronic pancreatitis (CP). Material and Methods: 80 patients with exacerbation of CP and 23 controls have been investigated. The level of reduced glutathione (GSH) and activity of enzymes of glutathione reductase (GR), glutathione transferase (GT), glutathione peroxidase (GP) have been determined in plasma and erythrocytes, while the MDA has been studied in plasma. Results: The MDA levels in plasma were higher in CP patients as against the values of the controls. The MDA level correlated positively with intensity of pain. Increased GSH concentration and activity of GT were revealed in plasma. In the erythrocytes reduction in GP activity and GSH concentration were determined. Conclusion: In the exacerbation of chronic pancreatitis lipid peroxidation and reduced antioxidant protection have been activated.
