The role of doctors in the formation of preventive compliance in patients of cardiological profile

Summary:

The purpose of the study is to analyze the compliance of cardiological patients to participate in the prevention of coronary risk and the role of doctors in the compliance formation. Material and Methods. The base of the study was the outpatient medical organizations of the Kostroma region. The study design included a systematic coronary risk assessment using the SCORE European scale; randomization of patients into groups according to the risk degree; assessment of prophylactic patient compliance; analysis of patient compliance; assessment of patient potential; assessment of the relationship between the level of prophylactic compliance of patients with the level of coronary risk; assessment of the quality of compliance prevention by primary care physicians. Scope of the study: 400 patients with cardiac profile, 200 cases of dynamic observation of patients with high and very high levels of coronary risk. Results. We found that 32.4% of patients had a high level of compliance; 64.5% of patients had an average level of prophylactic compliance and 3.1 % of patients had a low level (p<0,05). We discovered the main parameters of reducing of prophylactic compliance of patients, determining factors, as well as the parameters for reducing the quality of compliant prophylaxis in the primary health care system. Conclusion. The obtained data became the theoretical basis for the development of the technology for coronary risk compliance prevention including 7 stages and 3 levels.

Summary:

The article explains the prospects of sociological interpretation of the phenomenon of compliance. The theoretical construct of the phenomenon of compliance is based on the allocation of social parameters of the therapeutic interaction as a particular social practice. The presence of the disease is seen as a sign of social, determined by the dynamics of social activity of the patient in the context of changes in the characteristics of social interaction. The author gives a sociological interpretation of the concept of compliance as the dynamic state of the area of therapeutic interaction. Substantiates the prospects of sociological reflection of compliance, which allows to carry out the social dimension of the conditions of stability/instability of the therapeutic interaction, predict and manage change behavioral strategies patients.

Summary:

The aim of the work is to examine the relationship of philosophical, sociological and psychological approaches to the problem of ТВ in the situation of the disease. Materials and Methods. Philosophical reflection on the phenomenon of tuberculosis as a disease was based on the existential and structuralist approaches. Sociological analysis included the use of quantitative and qualitative methodologies to explore the social factors or determinants of tuberculosis. Psychological instruments were used to identify internal potential of the disease, the level of viability, its impact on the treatment process, situation analysis of the disease as a stressful mental state. Results. It was noted that the level of vitality is almost equal to the reference values which were average results of a survey of patients with an active ТВ of men of the third department. Conclusions. It was concluded that the Vitality Test revealed the correlation between criteria of viability and patient's compliance, which could help in predicting the treatment effectiveness and management of patients.

Pharmacoeconomic parameters have been examined in patients with ischemic stroke who have suffered from arterial hypertension, depending on use of antihypertensive therapy. The role of antihypertensive therapy as a factor that significantly reduces the treatment costs and improves stroke outcome has been proved. The research results show the importance of integrated analysis of clinical and economical factors in the treatment of patients with arterial hypertension
