Local effect of vitamin D on the immune system of periodontium

Summary:

 Literature review is devoted to the analysis of scientific findings about extraosseous effects of vitamin D on the immune system of the periodontal tissues. It was found that the biological functions of vitamin D (VD) in the body are diverse.Along with the detailed study of the effect on metabolic processes in bone tissue, VD makes a significant contribution to a large number of processes that ensure the activity of the immune system. In addition, VD increases the production of "endogenous antibiotics". This allows us to consider VD as a therapeutic agent that prevents the development of inflammatory and destructive changes in the periodontal tissues in population with simultaneous slowing of bone loss and preservation of bone mass of the dentofacial system. Currently detected hidden hypovitaminosis VD in almost majority of the population is considered a leading factor in the development of periodontal disease and its prevention and treatment is one of the most important tasks of therapeutic dentistry.

Morphofunctional state of spleening under light desynchronosis in the experiment

Summary:

 Aim: to study the effects of light desynchronosis on the morphofunctional state of the spleen of white male rats. Material and Methods. The study of light desynchronosis was carried out on 48 male rats using the Light Light model with an illumination power of 300 Ix in the daylight and 500 Ix in the dark. The laboratory animals in the experiment were divided into four groups: the control and three experimental ones, in which they were exposed to light for 1 day, 10 days, 21 days respectively. In the course of morphometry of histological sections of the spleen, the following parameters were assessed: the area of white pulp lymphoid follicles, B- and T-clependent zones of white pulp, the number of germinative centers and immunoblasts in B- and T-zones. Results. On day 1 of the experiment, an increase in the area of the lymphoid follicle, periarterial and B-dependent zones, an increase in the number of immunoblasts and reproduction centers was registered. By the 10th day there was a partial restoration of these indicators relative to the control values. On the 21st day of the experiment, a decrease in the growth dynamics of all the morphometric parameters of the spleen was observed. Conclusion. Light desynchronosis induces by the Light Light model leads to the development of the pathology of the immune system with an extensive shift of all measurable indicators. The article presents the historical origin of the term "Koebner phenomenon". The literature data reflect the etiology, pathogenesis and epidemiology of isomorphic mechanisms provoking responses in lichen planus, psoriasis, scleroder-ma, syphilis. Variants of the Koebner phenomenon's classifications are given. The clinical cases of limited scleroderma after mechanical injury are described.

Summary:

Research objective: to search new laboratory approaches to the diagnostics of immune forms of reproductive failures. Materials and methods. Retrospective research a case — control of 54 married couples with idiopathic reproductive failures (in the anamnesis — 3 and more spontaneously interrupted pregnancy in the 4-8th weeks) and 47 married couples having two and more children has been conducted. Results. It has been revealed that at the immune form of reproductive failures increase of cells of level A- mononuclear cells, expression of HLDR takes place that promotes tolerance cancellation to allogenic germs and to immune interruption of pregnancy. At reproductive failures female au-toserum positively influences activation of T-lymphocyte (CD3 +/HLADR +) that may lead to the death of half- allogenic germ. Conclusion. Level of expression of CD3 and HLADR on CD45 + of mixed allogenic mononuclear cells of spouses may serve as a diagnostic significant criterion for revealing immune reasons of reproductive failures.

Pathogenesis of oral hygiene, smoking, gestosis, immunosuppression and cytokine disbalance affect inflammatory periodontal diseases in pregnant women. The research goal is to study clinical and immunological features of parodentium and cytokine profile in oral cavity of pregnant women. Material and methods: The condition of parodentium tissues was studied at 240 women with physiological pregnancy and 360 with pregnancy complicadet by gestosis. Re-sults: Freguency and character of inflammatory periodontal diseases in pregnancy were defined. Immune and cytokine disbalance contributed greatly to pathogenesis of inflammatory periodontal diseases at pregnant women. Conclusion: Violations of immune homeostasis at the pregnancy, complicated gestosis, authentically are more expressed that contributes to the development of gingivitis and periodontal disease. The imbalance of cytokines can serve in oral liquid as additional diagnostic and predictive marker of severity of a course of inflammatory periodontal disease.

Correction of immune disorders in patients with malignant tumors at level of craniovertebral junction and upper cervical spine

Summary:

Objective: To study the cellular and humoral immunity in patients with malignant tumors of the upper cervical spine. Material and methods: The study included 22 patients with malignant tumors of the craniovertebral junction and upper cervical spine. To assess the function of the immune system the methods of population and subpopulation immuno-phenotyping of peripheral blood lymphocytes have been applied. Cytokine parameters and functional status (cytotoxic) activity of NK-cells have been studied using immunofermental methods. The indices have been presented in absolute and relative terms. All the patients have been subjected to the adjuvant therapy. Results: Correction of cellular immunity has led to significant increase in CD3 + (T-lymphocytes) and CD3 + CD8 + (T-killers). Cytokine levels in the groups studied in the dynamics have not been significantly altered. The changes of cytotoxic (functional) activity of NK-cells in the first group after the combined treatment have been considered to be higher than before the treatment. Cytotoxic index of (functional) activity of NK-cells in the second group under the study after the combined treatment has been determined to be lower than before the treatment. Conclusion: It has been found out that the application of complex treatment accompanied by immunotropic therapy may positively influence the immune status.

Evaluation of clinical efficacy of immunocorrection therapy for curing oral cavity candidosis

The research goal is to increase the efficacy of oral cavity candidosis treatment. 97 patients with oral cavity can-didosis have been examined and treated. Diagnostics of germ culture, microscopical investigation of smear of oral mucosa and the sensitivity determination of causal agents to antifungal drugs have been performed. In order to assess immunological status of patients the concentration of immunoglobulins in their saliva has been determined. In order to find out specific antibodies for yeast-like fungi immune-enzyme blood analysis has been used. All the patients have received complex treatment. The control group has been administered immunomodulator «Gepon» for application to oral mucosa. 64.8% of patients with chronic oral cavity candidosis have had low concentration of immunoglobulin G in their blood serum. The concentration of immunoglobulins IgA, IgG and IgM in oral fluid has been lower than normal. The treatment efficiency has been considered as a way of recovery of local immunological status. Local administration of «Gepon» has been proved to increase the treatment quality and remission period

Summary:

The research goal is to study clinical and immunological features of parodentium and cytokine profile in oral cavity of pregnant women.
The condition of parodentium tissues was studied at 200 women with physiological pregnancy and 300 women with pregnancy complicated by gestosis. According to the results of examination 50 women with gestosis and 50 women with physiological pregnancy had inflammatory periodontal diseases. Phenotyping of lymphocytes by immunofluorescence method, investigation of necrosis containing factor of tumour-a, interleukin-8, interleukin-4 and transforming growth factor beta-1 in oral cavity by immunofermental analysis were performed.
Frequency and character of inflammatory periodontal diseases at pregnancy were defined. Correlation of gingivitis and periodontitis at pregnancy with extragenital pathology was demonstrated. Immune and cytokine disbalance contributed greatly to pathogenesis of inflammatory periodontal diseases at pregnant women.
Thus pathogenesis of oral hygiene, smoking, gestosis, immunosuppression and cytokine disbalance affects inflammatory periodontal diseases at pregnant women
