Informativeness of optical coherence tomography in differential diagnosis of central chorioretinitis and age-related macular degeneration

Summary:

Purpose: the analysis of the informativeness of optical coherence tomography as an additional method in the diferential diagnosis of central chorioretinitis and age-related macular degeneration. Material and Methods. Studies conducted in 44 patients. The frst group included 20 patients with central chorioretinitis. The second group consisted of 24 patients with diferent forms of age-related macular degeneration. The informativeness of optical coherence tomography was analyzed in the diferential diagnosis of central chorioretinitis and AMD. Results. Uncomplicated forms of AMD and central chorioretinitis have a diferent tomographic picture. Using the OCT method it is possible to judge the change in the thickness of the choroid which increases in the acute phase of chorioretinitis. In the atrophic form of AMD, the thinning of the choroid extends over a considerable distance. OCT provides additional information on the localization of the infammatory focus. Conclusion. OCT-diferences were found at the level of the pigment epithelium, retinal photoreceptors and choroid in infammatory processes and degenerative diseases, which can help in difcult cases of diferential diagnosis. However, secondary choroidal neovascularization does not have specifc OCT signs in various diseases, since it is a complication of the main process.

Evaluation of retinal neuroarchitectonics in presence of optic nerve head drusen

Summary:

Purpose: assessment of retinal neuroarchitectonics in presence of optic nerve head (ONH) drusen. Materials and Methods. 108 patients (216 eyes) with bilateral drusen of the optic disc aged 18–60 years were examined, and the control group consisted of 28 healthy subjects. All patients underwent optical coherence tomography (OCT) using Cirrus HD-OCT 5000–7544 device (Carl Zeiss Meditec Inc.). Results. Three types of ONH drusen localization relative to the Bruch membrane were identifed: superfcial, deep and difuse. Difuse localization of the drusen was determined in 48.6 % of cases, deep — in 40.3 %, superfcial — in 11.1 %. Deep optic disc drusen were more common in a young age (35.5±13.1 years), difuse — in medium age (42.0±13.2 years), superfcial — at an older age (51.7±12.8 years). With superfcial and difuse drusen of the optic disc, a signifcant (p<0.05) decrease in the thickness of the ganglion cell (GC) layer in all studied sectors, a decrease in the minimum and average GC thickness was found, as well as a signifcant (p<0.05) decrease in the average thickness and cubic volume of the neuroepithelium. With deep drusen opacities, the changes in GC were less pronounced and statistically insignifcant. Conclusion. analysis of the obtained data showed that deep drusen of the optic disc are the initial stage of congenital pathology, difuse drusen are a developed stage, and superfcial drusen of the optic disc characterize the development of fnal stage.

Summary:

The development of optic neuropathy in leukemia and lymphomas has a wide range of etiological factors, being a result of neoplastic, infectious, compression, ischemic and toxic damage. Patients in the older age group may have cardiovascular risk factors that increase the risk of developing lesions of the optic nerve. Timely diagnosis of optic neuropathy in hemoblastosis and the choice of the optimal treatment method allow to maintain high visual function and improve patients» quality of life. The paper presents a clinical case of unilateral ischemic neuropathy in patients with clinical and hematological remission of acute myeloblastic leukemia and the presence of cardiovascular risk factors, as well as heart failure.

Wide-feld retinal optical coherence tomography for imaging of peripheral vitreoretinal interface and stratifcation of risk for rhegmatogenous retinal detachment recurrence

Purpose: to evaluate the peripheral vitreoretinal interface changes after combined laser-surgical treatment of local rhegmatogenous retinal detachments revealed by wide-feld OCT. Material and Methods. 21 patients (21 eyes) with local rhegmatogenous retinal detachment localized in the upper half of the fundus were enrolled in this study. All patients underwent a combined laser-surgical treatment, which included 3 steps: 1) Nd: YAG laser excision of the vitreoretinal adhesion zone, determined by wide-feld OCT; 2) pneumatic retinopexy; 3) barrier laser photocoagulation at the edge of the tear after retinal reattachment. All patients underwent OCT of the peripheral vitreoretinal interface to identify areas of pathological vitreoretinal adhesions in the post-operative period. Results. OCT analysis of 21 patients has demonstrated the complete elimination of the traction component in the treatment area. However, vitreoretinal adhesions near the tear area were found in 4 patients. Hormonal and lipid imbalances in the formation of myopic chorioretinal neovascularization in women

Purpose: to determine the role of hormonal and lipid imbalance in the formation of myopic chorioretinal neovascularization in women. Material and Methods. The frst clinical group includes the results of the study of patients with the active form of myopic chorioretinal neovascularization, the second — the results of the study of their paired eyes. The control groups were formed separately. Ophthalmologic and somatic status of all patients and people in the control groups were thoroughly examined. Results. An increase in the level of prolactin to 501.7±28.2 mlU/ml was established, in the control group to 272.9±33.4 mlU/ml, the level of follicle-stimulating hormone 13±0.1 mlU/ml, the control group 4,6±1.2 mlU/ml. The concentration of estradiol is reduced 35.2±3.1 pg/l, progesterone 2.8±0.1 nmol/l, compared to the control group 66.8±8.1 and 6.2±1.1 nmol/l respectively. An increase in the level of acute-phase proteins was revealed: fibrinogen 3.8 (14.0±0.5) and C-reactive protein 2 times (5.72±2.30) compared with the control group 3.6±0.5 and 2.5±0.7. Increased lipoprotein level (a) 50.2±1.2 g/l and ApoB/ApoA 0.6±0.1 compared with the control group 9.1±1.1 et 0.4±0.1 g/l. Conclusion. Purpose: to assess the diagnostic capabilities of optical coherence tomography (OCT) in En Face mode for planning and conducting surgical navigation using Navilas 577s laser system in patients with central serous chorioretinopa-thy (CSC). Material and Methods. 12 patients (12 eyes) with CSC underwent high-resolution OCT in En Face mode showing morphological changes in the retinal pigment epithelium (RPE). The treatment was performed on Navilas 577s laser device with an individual selection of the necessary energy parameters in the micropulse mode. The follow-up was performed at 1, 3 and 6 months. Results. According to the OCT data in the En Face mode, all patients had morphological changes in RPE, which made it possible to plan and perform computer-assisted laser surgery. 1, 3 and 6 months after treatment, positive dynamics was observed in all cases. Conclusion. The OCT method in En Face mode allows detailed study of architectonics and identifes pathological changes in RPE, which play an important role in the manifestation of CSC, as it enables planning and implementation of topographic-oriented treatment using the Navilas 577s navigation laser system in the areas of RPE detachments.

The far long-wave ultraviolet lichen planus therapy with the use of clearing agents

Summary:

 The goal: to increase the effectiveness of far long-wave ultraviolet therapy in patients with lichen planus using immersion agents. Materials and Methods. The study included 50 patients aged from 18 to 65 years with typical and hypertrophic forms of lichen planus who had received UVA1-therapy. All the patients were divided into 2 groups: in the 1-st group before the phototherapy on the elements of lichen planus caused an clearing agent propylene glycol (PG) and in the 2nd group PG was not used. The dynamics of the enlightenment of the skin was assessed using optical coherence tomography. After the end of treatment was a decrease in mean group values of ILP 96% of the original value, pruritic index 98%, DLQI 87% respectively. Results. In the 1-st group patients occurred more rapid resolution of the pathological process, the rate of phototherapy was 15-20 procedures. In the 2nd group to complete resolution of the pathological process, required an increase in the number of procedures to 20-25. Conclusion. Far long-wave UV therapy showed high efficacy and compliance in patients with lichen planus, the use of the 1,2-PG allowed to shorten the duration of treatment of patients.

Summary:

Diabetic macular edema (DME) is a leading cause of blindness in patients with diabetes. The purpose of the research is to evaluate the effectiveness of treatment of patients with DMOs resistant to the administration of ranibi-zumab, using the intrauritreal implant "Ozurdex". Material and Methods. 66 patients (98 eyes) are examined. All of the patients were divided, into three groups, depending on a stage of a diabetic retinopathy. The patients underwent a complex of examinations: along with traditional methods, optical coherence tomography, fluorescence angiography of the fundus, multifocal electroretinography, determination of the content of VEGF-Aand cytokines in serum were used. Results. According to ОСТ of a retina and a visometry there are differences in efficiency of treatment depending on a condition balance of cytokine. Conclusion. The use of the intrauretic implant "Ozurdex" was more effective in patients with DMO in the stage of p re-pro I iterative diabetic retinopathy and in patients resistant to anti-VEGF therapy.

Summary:

Objective: to develop algorithms of processing of video images of optical slices of the eye retina to quantify the degree of folding of the epiretinal membranes and of the central fossa. Material and methods. The object of the study was the video image of the retina obtained by optical coherence tomography. To develop methods of determining the degree of folding epiretinal membrane was formed mathematical model of the profile consisting of a base profile (low frequency component) and folding (high frequency part). Results. There had been developed two alternative methods of estimation of folding epiretinal membrane: retinal-averaging method and the method using the wavelet transform. The algorithm of geometrical parameters of the central fossa: the height, width and line shape. These algorithms are implemented in a software system. Conclusion. The practical application of the developed system showed its adequacy, as well as an introduction into medical practice the use of quantitative estimates of some parameters of the retina condition.

Summary:

Aim: working out the methods of visualization of information obtained during optical coherent tomography in normal skin and in series of inflammatory disorders. Materials and Methods. OCS1300SS (made in Thorlabs, USA) was used in which the source of emission of radiation was a super-luminiscent diode with mean wavelength of 1325 nm. 12 patients with different skin conditions and 5 virtually healthy volunteers were examined with ОСТ procedure in OPD and IPD settings. High resolution USG numerical system DUB (TPM GmbH, Germany) was used for comparative USG assessment. Results. ОСТ demonstrated considerably more detailed picture of the objects scanned compared to USG investigation. Image obtained with the help of ОСТ contains vital information about sizes of macro-morphological elements, status of vascular elements and their density in different depths of the skin. Conclusion. Additional results obtained from ОСТ of the skin lesions in plane section improves attraction for ОСТ in practical dermatology.
