Efficacy of surgical treatment of renal form of hyperparathyroidism

Summary:

Objective: to study the efficacy of surgical treatment of primary hyperparathyroidism in risk of uronephrolithiasis progression. Material and Methods. The research involved 55 patients, the biochemical and clinical parameters before and after treatment with retrospective-prospective method of analysis were studied and analyzed. Results. It is established that the surgical treatment of patients with uronephrolithiasis is proved to be effective in 96,4% of the cases, as it allows to eliminate the classic symptoms of primary hyperparathyroidism, and urolithiasis. Improvement of laboratory hormonal and biochemical indices of mineral metabolism, and, consequently, reducing the severity of risk factors of urolithiasis is marked. Conclusion. Surgical treatment of renal forms of primary hyperparathyroidism is a pathogeneti-cally reasonable method. Parathyroidectomy performed in an adequate amount prevents relapses and progression of diseases that are the consequence of primary hyperparathyroidism.

Summary:

A review of the literature is devoted to the most topical diagnostic questions and treatment of patients with urolithia-sis. The role and the latest advances and benefits of minimally invasive surgery were described. Particular attention was paid to the problems of residual calculi and the condition of the renal parenchyma in the perioperative period. The theme of choice of conducting such patients tactics was considered. The actual diagnostic methods and a differentiated approach to the algorithm choice of the examination and treatment of patients with staghorn nephrolithiasis were considered. The review provided a scientometric analysis of research information (documents reference), contained the analytical array of scientific publications. Electronic information resources of scientific electronic library bibliographic databases: eLibrary, PubMed, Clinical Key, Science Direct — were used.

Summary:

 The review is devoted to current state of the problem of primary hyperparathyroidism. One of the important steps in the study of pathology of parathyroid glands and their decision value in development of primary hyperparathyroidism was introduction of automated screening of the ionized calcium of plasma of blood to all patients asking for medical care. Understanding of the nature of this process allowed the doctors of many specialities to unite the efforts in the analysis of results of the already accumulated experience and extend possibilities of early preclinical exposure of this disease.

Correlation between size and density indices of concrements in patients with nephrolithiasis and effective method of treatment by distance shock-wave lithotripsy

Summary:

The aim of the article: evaluation of the results of treatment of patients with nephrolithiasis by distance shock-wave lithotripsy on the basis of the data on kidneys received from multispiral computed tomography. Material and Methods. The study involved 46 patients who were hospitalized in the clinic of urology with diagnosed urolithiasis with the pres- ence of a renal calculus from 10 to 17 mm in size. A complex of laboratory analyses including multispiral computed tomography was used. The concrement elimination was carried out by distance shock-wave lithotripsy Results. Correlation analysis and ROC-analysis did not show the efficacy of distance shock-wave lithotripsy comparing scanning procedures and distance shock-wave lithotripsy. Conclusion. The size and average density of a concrement according to multispiral computed tomography should not be served as a basis for prognosis of effective fragmentation of concre-ments by distance shock-wave lithotripsy in patients with urolithiasis caused by stones of 10-17 mm in size.

Biomolecular assessment of renal function in various types of surgical treatment of renal cell carcinoma

Summary:

Objective: to assess the possibility of using the markers of acute kidney injury to predict the preoperative risk for the subsequent decline in glomerular filtration rate in different types of surgical treatment of renal cell carcinoma (RCC). Material and methods. 60 patients with histologically confirmed RCC T1-3M0N0 operated in the clinic of urology of Saratov State Medical University n.a. V. I. Razumovsky in the volume of nephrectomy or partial nephrectomy. Before surgery all patients underwent standard examination intended for patients with kidney tumors: ultrasound, MRI, excretory urography and dynamic renoscintigraphy, the perioperative values of serum creatinine and glomerular filtration rate were determined. Using the method of immuno-enzymatic analysis the concentrations of excreted with the urine NGAL and IL-18 in serum samples at the preoperative stage, after 5 days and after 1 month of postoperative follow-up were investigated. Differences in clinical data and clinical variables were compared by using Spearman rank correlations and t-test. Results. The determined parameters of acute kidney injury markers IL-18 and NGAL in the early postoperative period were increased in patients after open nephrectomy. Moreover, the correlation analysis according to the Spearman method revealed a strong significant correlation between the preoperative levels of IL-18 and GFR after surgery (r=1; p<0.05). Conclusion. Laparoscopic resection of RCC is the method of choice for surgical treatment of RCC. As a predictor of adverse prognosis, the level of IL-18 in serum may be used. Its increase correlates with a decline in renal function in the postoperative period, and according to some reports, IL-18 also is an independent predictor of adverse prognosis in patients with a localized RCC.

Direct renin inhibitor aliskiren in surgical treatment of nephrolithiasis

Summary:

Research objective: The study investigates the antifibrotic nephroprotective effects of aliskiren on the condition of renal hemodynamics, changes of profibrotic cytokines level and markers of renal damage in patients with nephrolithiasis undergone various types of surgery. Materials and methods: Patients have been divided into 2 groups. The investigations have been performed before the surgery and in a month period. Concentrations of TGF-p, markers of acute renal tubulointerstitial damage — NGAL and p2-microglobulin in urine, levels of ET-1 in serum and indicator of endothelial dysfunction have been determined. Assessment of renal blood flow state by Doppler ultrasonography has been carried out in the same terms. Results: As a result of aliskiren treatment the decrease of mediators of tubulointerstitial damage and nephrofibrosis, significant improvement in renal blood flow in a month period after the surgery have been marked in comparison with the group which had not received nephroprotective therapy. Conclusion: Direct renin inhibitor aliskiren may be used as a means of renal protection therapy in treatment of patients with nephrolithiasis.

Summary:

The aim of this study was the estimation of clinical importance markers of renal damage at surgical treatment of nephrolithiasis. We studied results of operations in 110 patients with nephrolithiasis. The study presents new candidates for biochemical markers of acute kidney injury and renal interstitial fibrosis, with potentially high sensitivity and specificity. In this study were measured the concentrations of these biomarkers in urine. We suggested, that increased concentrations molecular mediators of renal damage (L2, IL-18) and interstitial fibrosis (MCP-1, TGFp, IGF1) in the patients with nephrophibrosis and strongetubulointerstitial injury, saved long-time period after operation, has been shown of progression chronic kidney disease

The aim of the study is to evaluate the influence of angiotensin converting enzyme (ACE) inhibitors of quinapril on the condition of renal hemodynamics, changes of profibrotic cytokines level and transforming growth factor at patients with nephrolithiasis undergone various kinds of surgery. Patients were divided into 2 groups initially and in 1 month after surgery. Concentrations of IL-6, TGFp and МСР-1 were determined in blood serum. Assessment of renal blood flow state by Doppler ultrasonography was performed in the same terms. As a result of quinapril treatment the decrease of МСР-1, TGFp and IL-6 was marked in comparison with the group which did not receive nephroprotective therapy. The analysis of Doppler ultrasonography data showed authentic improvement of renal blood flow in 1 month after surgery. The most informative parameter proved to be the resistance index that indicated the decrease of vascular resistance after surgical treatment in condition of quinapril therapy
