Quality of medical care for the elderly in out-patient conditions according to the sociological survey

Summary:

The process of aging is caused by the early development of chronic complications and diversity of morphological and functional changes of organs and systems. Therefore the provision of preventive and curative care of this category of citizens has significant clinical and organizational features. Aim: to identify the main problems of medical care for the elderly and to examine the medical care. Material and Methods. 250 physicians of various specialties of medical institutions of the Saratov region on health care of the elderly in out-patient clinics have been surveyed by specially designed questionnaires. The state of receiving care in out-patient clinics has been studied among the elderly and the senile patients (n=568) by questioning. Results: The majority of physicians recognize the importance of the problem of increasing the quality of care for the elderly. The development of assistance of gerontological profile and the need for optimization of rendering out-patient care for this category of population in the central clinics and regional medical institutions. Conclusion. The level of satisfaction of elderly patients depends on the age characteristics, gender, health and social status and the place of residence of the study group.

Management of adaptation of graduates of medical schools to conditions of independent professional activity: research and optimization

Summary:

The Objective: research of regularities of adaptation of graduates of medical schools to conditions of independent professional activity and justification of the directions of optimization of management by this process. Material and Methods. Object of research included functioning of system of adaptation of graduates of medical schools to conditions of independent professional activity. Are carried out: The study of reports of the Saratov region for 2006-2012, documentation of 16 treatment-and-prophylactic medical organizations and 84 responses on graduates of Saratov State Medical University n.a. V. I. Razumovsky; anonymous retrospective questioning of 164 doctors after professional retraining at the faculty of professional development; expert questionnaire of 15 persons of the faculty of organizational chairs have been carried out. Results. In the work "complex adaptation factor"; dynamics of level of social and psychological, psychophysiological, organizational and professional indicators of adaptation of graduates to conditions of independent professional activity; the characteristic of "lagging behind" doctors; purposes of management of adaptation, importance of stages of its organizational support have been established. The ways to evaluate the success of individual adaptation and management of this process have been worked out, which are designed on the basis of the corresponding authorized optimization technology. Conclusion. Results of the conducted research allowed to expand idea of adaptation of graduates of medical schools to conditions of independent professional activity and to solve a number of applied problems of its optimization.

Organizational and economic parameters of resource provision of medical organizations

Summary:

The aim of the research is to determine the main patterns of activity of in-patient medical organizations according to the resource provision of medical and diagnostic process. Material and methods. 20 medical organizations, including regional organizations, central regional hospitals, Saratov municipal health care organizations, non-private and private medical organizations have been surveyed. The survey of 60 chemists of hospital pharmacies has been conducted. Results. The analysis of the survey has showed the resource provision of medical and diagnostic process by medical products, the degree of technological provision in hospitals of various types and stated the main problems of activity on resource provision. Conclusion. It has been found out that the current medical provision with medical products is connected with the action of considerable number of objective and subjective factors and it defines the efficiency of medical social activity.

Improving of a training system as a model for continuing education in rehabilitation

Summary:

The presented model of continuing education in the field of rehabilitation training system provides physicians clinicians, allows to maintain the necessary level of their professional competence. Nowadays the need to improve constantly, to develop and complement forms of advanced training is connected with the active introduction of scientific and technological progress into medical practice, leading to the improvement of medical technologies.

Scientific substantiation for the analysis of medical and social aspects of a healthy lifestyle as a basis of the effectiveness of rehabilitation in the modern conditions

Summary:

Efficiency of performed rehabilitation events in many respects depends on a state of health of the person which, in turn, directly depends on his/her medico-social activity. Aims: analysis of medico-social aspects of a healthy lifestyle of medical workers and patients of the center of recovery medicine and rehabilitation. Material and Methods. According to specially developed anonymous questionnaires there had been the research of the personnel (410 questionnaires) and patients (300 questionnaires) of the Center of recovery medicine and rehabilitation of Federal State Budgetary Institution Medical and Rehabilitation Center of Ministry of Health of Russia (TsVMR). Results. It is shown that the majority of participants regardless of professional functions and responsibilities understand an important role of healthy food and try to observe the correct diet, however, medical workers take this component of a healthy lifestyle more seriously. Thus the most part of citizens not having a direct bearing on health system are reluctant to care abot questions of healthy food, this emphasizes insufficiency of sanitary and hygienic education. Conclusion. Results of research can be used for development of scientific bases of increase of the efficiency of rehabilitation and recovery treatment in modern conditions.

Monitoring of patient's opinion on the quality and availability of rehabilitation organization in modern conditions

Summary:

Aim: to study the patients opinion of the center of restorative medicine and rehabilitation of «Medical rehabilitation center» of MOH of Russia: the organization and availability of rehabilitation and restorative treatment. Material and Methods. Anonymous survey of 490 patients using the specially designed questionnaire. Results. The most of the respondents, almost 3A (76,1%) gave the «good» and «excellent» marks, 19,8% — «satisfactory», 4,1% — «unsatisfactory». Conclusion. Results of this research can be used by health care regulatory bodies when developing the administrative decisions directed on improvement of availability and quality of rendering this specialized medical care.

Medico-social characteristics of patients treated in rehabilitation centres

Summary:

Aim: analysis of the medical social characteristics of patients of rehabilitation center. Material and methods. The data that had been got by extracting from medical records of patients of the Center of restorative medicine and rehabilitation «Medical rehabilitation center» MOH of Russia on a specially developed «Patient card» were analyzed. Results. Among the majority of patients (who got treatment in the center) with diseases of the musculoskeletal system and connective tissue (44,9%), almost 1/3 (29,2%) were patients with diseases of the circulatory system, in the third place — patients with injuries (14,1 %). Conclusion. As a result of the peculiarities of a patient health condition of different age and social position had been revealed.

Analysis of major trends of morbidity of population as a base of necessity for organizing rehabilitationc

Summary:

The study aims the research of morbidity in Moscow and the Moscow Region in those diseases rates for which it had been required further rehabilitation and restorative treatment, against the background of the analysis of similar indicators in the whole of the Russian Federation in the dynamics for eight years. The necessity of enlargement restorative treatment and rehabilitation for citizens, including databases of major medical centers had been reasoned.

Analysis of medical institutions with various organizational forms of rehabilitation treatment and outpatient departments

Summary:

The study aimed organization of the analysis of activity of two medical institutions rendering services in rehabilitation and recovery treatment to adult population, having various organizational forms. Material and Methods. For five years there had been studied practical experience of rendering medical care on recovery treatment and rehabilitation to adult population on the basis of medico-statistical processing of reporting documentation of the Medicine Recovery Center and the rehabilitation, Ministry of Health of Russia functioning on the basis of the "Medical and Rehabilitation Center" — large versatile medical center and the interdistrict center of recovery treatment on the basis of city policlinic of St. Petersburg. Results. As a result it had been established an advisability of rendering this type of specialized medical care by medical institutions with different organizational forms. Conclusion. The interdistrict centers of rehabilitation functioning as a part of city policlinics, are undoubtedly necessary, as the closest medical setting for patients' homes, however such functions as diagnostics of a functional condition of an organism and an objective assessment of a state of health of patients with use of screening techniques; inspection of the organized collectives and groups of the population for the purpose of identification of groups of risk, establishment of extent of influence of environmental factors on a state of health, active supervision over persons with factors of the increased risk of diseases and correction of the revealed functional violations, etc. can be performed only in the centers organized on the basis of modern versatile treatment-and-prophylactic establishments, equipped with the modern diagnostic devices, allowing to supplement traditional methods of drug therapy with new effective techniques of treatment.
