Summary:

Purpose: to develop optimal approaches to the use of cytofavin for neuroprotection and to evaluate their efective-ness in patients after endovitreal surgery for rhegmatogenous retinal detachment (RRD) with anatomically complete retinal attachment. Material and Methods. 68 people (68 eyes) with primary RRD. Three-port vitrectomy with silicone oil tamponade in the vitreous cavity and its subsequent removal were performed. The patients were divided into 2 groups: the main group (38 people who received cytofavin added to standard therapy), and the control group (30 people with standard therapy only). Depending on visual prognosis calculated before surgery, the main and control groups consisted of patients with tolerable and low levels of visual prognosis. We used laser doppler fowmetry (to register microcirculation index (MI) and microcirculation efciency index (MEI)). Morphological signs of macular ischaemia were recorded by optical coherence tomography angiography (OCT-angiography). Results. Regardless of visual prognosis, the response dynamics of the chorioretinal microcirculation in patients was unidirectional. In patients of the main group 6 months after the surgery, more signifcant positive changes in growth of amplitude of MI and MEI, as well as positive dynamics of OCT-angiography were observed. Conclusion. Including cytofavin neuroprotection in postoperative treatment in patients after

Organization: The S. Fyodorov Eye Microsurgery Federal State Institution
Summary:

Purpose: to evaluate the peripheral vitreoretinal interface changes after combined laser-surgical treatment of local rhegmatogenous retinal detachments revealed by wide-feld OCT. Material and Methods. 21 patients (21 eyes) with local rhegmatogenous retinal detachment localized in the upper half of the fundus were enrolled in this study. All patients underwent a combined laser-surgical treatment, which included 3 steps: 1) Nd: YAG laser excision of the vitreoretinal adhesion zone, determined by wide-feld OCT; 2) pneumatic retinopexy; 3) barrier laser photocoagulation at the edge of the tear after retinal reattachment. All patients underwent OCT of the peripheral vitreoretinal interface to identify areas of pathological vitreoretinal adhesions in the post-operative period. Results. OCT analysis of 21 patients has demonstrated the complete elimination of the traction component in the treatment area. However, vitreoretinal adhesions near the tear area were found in 4 patients. These patients underwent additional barrier laser photocoagulation at zones of pathological vitreoretinal traction. Conclusion. Visualization of

Epidemiological characteristics of rhegmatogenous retinal detachment in the Central Black Earth region

Summary:

Aim: to estimate the frequency of rhegmatogenous retinal detachment among the population of the Central Black Earth region. Material and Methods. A retrospective study on the study of patients with myopia, who turned to the rhegmatogenous retinal detachment (RRD) in "S. Fedorov Eye Microsurgery Federal State Institution" in the period from 2006 to 2017 The epidemiological characteristics of patients who have applied to the RRF from five regions of the Central Black Earth Region (Belgorod, Voronezh, Kursk, Lipetsk, Tambov) are presented. Results. There is an annual increase in the number of patients with myopia complicated by RRD. The highest number of cases of myopia complicated by RRD was recorded in the age group of 50-59 years and 60 years and older, the smallest — in the age group 0-19 years. The female population suffers from a myopia complicated by the RRD equally with the male, but in the age groups 20-29 years, 30-39 years, men suffer more often, in the age groups 50-59 years, 60 years and older, women suffer more often. Among the patients from the Voronezh region, the largest number of cases of RRD was registered in the age group of 50-59 years, among patients from Tambov, Lipetsk and Kursk regions — in the age category of 60 years and older, from Belgorod region — in the age category of 30-39 years. The majority is represented by pensioners (60.3%), working people — 20.8%, disabled people — 15.8%, others 3.1%. Conclusion. Among the population of the regions of the Central Black Earth region, there is an annual increase in the number of patients with RRD, while the number of such patients will only increase, which indicates the need to improve the organization of eye care to the public.

Surgical treatment of rhegmatogenous retinal detachment with inferior breaks using gas tamponade of the vitreal cavity

Year: 2018, volume 14 Issue: №4 Pages: 870-872
Heading: Ophtalmology Article type: Short message
Authors: Shkvorchenko D.O., Gorshkov I.M., Yukhananova A.V, Khurdaeva A.G.
Organization: The S. Fyodorov Eye Microsurgery Federal State Institution
Summary:

Purpose: to evaluate the efficacy of tamponade of a vitreous cavity with gas-air mixture in patients with rhegmatogenous retinal detachment (RRD) in the lower hemisphere. Materials and Methods. We examined and operated on 52 patients with RRD with the localization of the discontinuities in the lower hemisphere: 30 patients with RRD with involvement of macula and 22 patients with RRD without macular involvement. All patients underwent general and special ophthalmological examination. Standard surgical treatment of retinal detachment finalized with injection of gas-air mixture into the vitreal cavity was performed. Results. In all cases, patients with RRD without involvement of macula and in 27 cases of RRD with involvement of macula there was a full adherence of the retina. In 3 cases patients with RRD involving the macula underwent repeat surgery with tamponade of the vitreous cavity with silicone, after which the retina adherence was finally observed. Close relationship between the improvement of visual acuity in patients 6 months after the operation and reduction in retinal thickness. Conclusion. The obtained results demonstrate the possibility of using gas tamponade of the vitreous cavity in RRD surgery with localization of rupture in the lower hemisphere.

Surgical treatment of regmatogenous retinal detachment complicated by macular hole using platelet-rich blood plasma and local staining

Year: 2018, volume 14 Issue: №4 Pages: 867-870
Heading: Ophtalmology Article type: Original article
Authors: Zaharov V.D., Shkvorchenko D.O., Krupina E.A., Kakunina S.A., Norman K.S., Khurdaeva A.G., Yukhananova A.V.
Organization: The S. Fyodorov Eye Microsurgery Federal State Institution
Summary:

Purpose: to study the effectiveness of surgical treatment of regmatogenous retinal detachment (ROS) complicated by macular hole (MH) using platelet-rich blood plasma (PRP). Material and Methods. The study included 27 patients (27 eyes) with a diagnosis of ROS complicated by MH. In each case, the technique was used, namely, that after performing vitrectomy, a perfluororganic compound (PFOS) was injected into the hole area with a diameter of 2-3 diameter of the disk, to prevent penetration of dye under the retina, stained and removed internal limiting membrane (ILM). After serial PFOS/air replacement at the gap introduced PRP. The operation was completed with the introduction of silicone oil (CM). CM was removed from the vitreous cavity after 2 months. The follow-up period is 12 months. Results. In all cases, it was possible to achieve the adhesion of the retina and block Mr. During the observation period visual acuity increased to 0.1-0.9. Conclusion. According to the evaluation of the results, surgical treatment of regmatogenous retinal detachment complicated by MH using PRP is an effective method of treatment.
