Eradication therapy in children: the choice of the optimal treatment regimen

The review is devoted to the choice of the optimal treatment regimen of eradication therapy in children. Main tasks in the treatment of children with gastroduodenal pathology associated with Helicobacter pylori are eradication of the pathogen, restoration of the damaged mucosa and prevention of recurrence of the infection. A particular importance in the composition of eradication schemes is the choice of antibacterial drugs. The main requirements for anti-Helico-bacter therapy regimens used in pediatric practice are: eradication rate of at least 80 %, economic accessibility, good tolerability with minimal severity of adverse reactions, optimal frequency of admission. An analysis of a number of domestic and foreign studies has shown that insufcient adherence of the patient to therapy, the presence of antibacterial resistance, pathogenicity factors in a number of strains of H. pylori have a negative efect on the efectiveness of treatment and should also be taken into account when choosing therapeutic tactics. Successful eradication of H. pylori in most cases depends on the optimal treatment regimen also has an impact on the result.

The microecological and immunological problems of persistention of Helicobacter pylori

The interrelation of persistence of Helicobacter pylori in stomach with innate and adaptive immunity of human has been discussed. The analysis of the mechanisms of immunological recognition of the antigens of this microorganism and response to them has been performed. It has been considered, that the development and course of gastritis and ulcer in human with Helicobacter pylori stipulate for inadequate reactions of immune system and common reactivity by the individual organism in presence of Helicobacter pylori in mucus of stomach.

This article represents results of pharmacoepidemiological research of H.pylori eradication treatment of adults and children in Volgograd. The aim was to determine H. pylori pharmacotherapy stereotypes and compare with international experts' recommendations. Material and Methods. There were analyzed CRFs of 94 adults and 132 children with H.pylori associated diseases in Volgograd region. Results. There were identified mistakes of treatment regimes and eradication control principles. Conclusion. The treatment of H.pylori infected adults and children in Volgograd does not match to international experts' recommendations.

Immunological and morphological aspects of diagnostics of Helicobacter pylori infection and Herpesviridae viruses

100 patients with inflammatory diseases of gastrointestinal tract were examined. Microbial component of inflammation process was detected by means of polymerase chain reaction (PCR). By means of PCR etiological microbial factor of mononuclear cells in peripheral blood (MCPB) was investigated. Mixtinfication of mucosa membrane of gastrointestinal tract with H. pylori and Herpesviridae in pathology of gastroduodenal area and presence of fragments of microorganisms DNA in PBMC were revealed. This phenomenon may be important in pathogenesis of pathology of gastrointestinal tract and forms the basis for its further investigation

Pharmacological effects of immunomodulators produce normalization of immunochemical function of homeostasis. This phenomenon determines optimization of Helicobacter pylori eradication. The combination of immunomodulators with antibiotics has a double effect. Antibiotics destress the functional activity of microorganisms and increase it’s sensitivity to the effect of phagocytes. On the other hand immunomodulators increase functional activity of phagocytes to eliminate microorganizms Оptimization of Helicobacter pylori eradication by using of immunomodulators is necessary for clinical practice. It includes prophylaxis of recurrences and complications caused by drugs action on immune system in patients with chronic diseases.
