Summary:

The purpose: to analyze the prevalence of digestive system pathology among the population of different age groups in comparison with the heliogeomagnetic environment. Material and Methods. Based on official statistical reports of medical institutions and medical documentation of medical institutions of Saratov in the period 1990-2006, digestive system prevalence rates in different age groups were studied. A correlation analysis of these indicators with the average annual values of Wolf numbers is performed. Results. A strong negative relationship was found between the prevalence of ulcer perforation among the young population and solar activity; for the elderly and senile population, a strong positive relationship between the prevalence of acute cholecystitis and solar activity was revealed. Conclusion. Some parameters of the "gastroenterological" health of the population may be associated with solar activity, which differs in different age groups of the population.

Peptic ulcer in women and men: clinical, endoscopic and morphofunctional features in young and elderly patients

Summary:

Purpose: analysis of clinical, endoscopic and morphofunctional parameters of the course of peptic ulcer of the stomach and duodenum in women and men of young and old age. Material and Methods. 170 patients with peptic ulcer were examined. We used a clinical, endoscopic examination with a histological examination of biopsy with determination of H. pylori in the stomach by a histobacterioscopic method, estimated sex hormone receptors density by relative area (the ratio of the area of immunopositive cells to the total area of the drug). Results. The features of the course of the disease were found: age-related, gender-related, not related to gender and age, associated with the localization of an ulcerative defect (in the stomach or duodenum). It has been shown that among women more than among men, the number of patients with an atypical course of the disease increases with aging; gender differences in the course of the disease in the elderly are more pronounced with duodenal ulcer localization. Conclusion. Clinical, endoscopic and morphofunctional features of the course of peptic ulcer of the stomach and duodenum associated with the gender and age of patients were revealed.

Evaluation of influence of social factors on the psycho-emotional state of persons of elderly and senile age

Summary:

Objective: to identify patterns of influence of social factors on the psycho-emotional state of the elderly and senile age. Material and Methods. A survey of 385 patients (average age-78.5±0.5 years). The following research methods were used: sociological (questionnaires composed of questions relating to the psycho-emotional state from the social block of the WHO quality of life questionnaire), statistical (Chi-square and the coefficient of mutual conjugacy of Chuprov»s qualitative signs using SPSS). Results. In the course of the study it was proved that the psycho-emotional state of the elderly and senile age is influenced by age, marital status, education and the number of children. Life satisfaction among married people was 3.4, widows and widowers 3.3, divorced 2.9 points. The strongest relationship between the level of education and satisfaction of respondents with their ability to make decisions and satisfaction with themselves as a person was revealed in persons with full secondary education (3.6 and 3.2 points, respectively), the lowest — in persons with secondary special (2.9 points) and incomplete secondary education (2.6 points). Respondents with two children and more (1.8 points) who are married (2.0 points) feel less lonely. In general, there is a reassessment of the individual, social status has a huge impact on the well-being and lives of people. They begin to perceive everything that is happening around them more sharply. All this together affects the objective component of life. Conclusion. The regularities of the influence of social factors on the psycho-emotional state of the elderly and senile age. Satisfaction with life in old and old age depends on marital status, education and number of children, which is connected with the opportunity to receive help and help others, and, accordingly, a higher appreciation of oneself as a person.

Analysis of resources of therapeutic institutions providing medical care to patients with diseases of the circulatory system

Summary:

Objective: to conduct an analysis of the provision of beds and medical personnel of medical institutions in the region that render medical care to patients with circulatory disorders. Material and Methods. Calculation of the optimal volume of inpatient medical care, the required number of beds and staff units of cardiologists in the region was carried out in accordance with regulatory legal acts. Results. In medical organizations, an excessive number of round-the-clock beds on the "cardiology" profile (more for 186 beds) and insufficient number of cardiac beds in day hospitals (less than 40 beds) were deployed; the number of cardiologists working in cardiological rooms of polyclinic medical organizations, dispensaries for cardiology and daily hospitals has been identified. Conclusions: Redistribution in the region's medical organizations of the bed fund, material, and financial resources leads to the optimization of patients' referrals and promotes rational use of resources of health care, increasing the availability and improvement of quality of medical care to adult population of the region.

Issues on optimization of primary health care for working patients with diagnosed hypertension

Summary:

Aim: to justify the ways of optimization of availability of primary health care for working-age patients with arterial hypertension. Material and Methods. The study of the availability of drug treatment of working-age patients with arterial hypertension was based on a survey of 409 patients and calculated the average cost of treatment of this group of patients for 1 month in accordance with the care standards. Results. On 01.06.2016, the cost of treatment for a patient with stage 1 hypertension amounted to 316.89 rubles per month, with 2 stage 652.68 rubles per month. 67% of patients of working age are unable to follow the doctor's recommendations and take the prescribed medicines. Conclusion. Provision of free medicines to patients with arterial hypertension of stages 1 and 2 and the formation of their commitment to preventive treatment causes a direct economic effect, which will be 7915979 rubles per year, and indirect economic effect of the decreased mortality from cardiovascular diseases, disability, reduce in disability days and increase of the gross domestic product.
