Peculiarities of psychological tests for pregnant women prone to abandoning a newborn

Summary:

The goal is to develop and use psychological tools (test) as an element of the system of primary prevention of early social orphanhood being created in a large industrial region. Material and Methods. The developed psychological test was empirically tested in 2018 on a group of pregnant women, consisting of 113 respondents aged 15-40 years, living in the Chelyabinsk region. The electronic version of the test (questionnaire) was introduced into the work of the regional obstetric and gynecological service in September 2019. Results. The questionnaire showed high sensitivity, identifying in 95% of abandoned mothers in 2019 an increased tendency to abandon a newborn. Most of the pregnant women inclined to abandon the child were identified in urban districts of the region (51 women), and the least — in rural areas (17 women). During the year, a positive effect in the prevention of abandonment was achieved in relation to 54.0% of women initially inclined to abandon a newborn. The best result was shown in the regional center — 60.0%. Conclusion. An effective diagnostic technique has been obtained, which makes it possible to purposefully carry out preventive work to prevent early social orphanhood. The developed test made it possible to exclude as much as possible the subjective moment in the statements of pregnant women about the fate of newborns, which led to a more expeditious start of preventive measures to counter refusals.

Clinical significance of cystatin С and TGF-01 in the assessment of renal function in the surgical treatment of renal cell carcinoma

Summary:

Objective: to evaluate the possibility of using cystatin С and TGF-p as prognostic markers of renal impairment and unfavorable oncological results of surgical treatment of renal cell carcinoma (RCC). Material and Methods. We examined 60 patients with RCC. Patients were divided into 3 groups: with RCC after nephrectomy (group 1); after laparo-scopic nephrectomy (group 2); after laparoscopic resection of the tumor (group 3). All patients before the operation on the 7th-10th day, after 1-3 years of postoperative follow-up, the concentrations of cystatin С and TGF-p in the blood serum were studied by solid-phase ELISA. Results. The majority of patients in the groups showed a significant decrease in glomerular filtration rate (GFR) compared to the control, and 3 years after nephrectomy, most patients had stage C3A chronic kidney disease (CKD). The Cystatin С content in all groups of patients with RCC had a statistically significant increase compared to the control (2664, 3103, and 2709 ng/ml in groups 1.2 and 3 versus 143 ng/ml, respectively) (p<0.05). Conclusion. In the majority of patients with RCC, postoperative initiation and progression of "surgical" CKD occurs, which persists 3 years after surgical treatment. Cystatin С can be considered as a predictor of high antitumor activity in patients with RCC, and requires monitoring in the long-term postoperative period.

Summary:

Differential diagnostics of epileptic paroxysms is often complicated and requires a multidisciplinary approach. Clinical presentation of epileptic and non-epileptic paroxysms is polymorphic and correct diagnosis requires clear understanding of semiotics and etiology of these conditions. The article presents a clinical case of a complicated course of the intermediate period of TBI with SIADH and seizures which mimic posttraumatic epileptic seizures.

Somatic sex determination of an adult by bone remains (review)

Summary:

The article presents a comprehensive osteoscopic approach based on the visual identification of high- quality semi-diagnostic signs on the bones. The differences in the bones of the skeleton in this approach are determined by the greater development of the muscles in men as compared with women, as well as the performance of a fertility function by the woman. On the pelvic bones, differences are functional in nature: the structure of the male pelvis is determined only by supporting and motor functions, and the structure of the female pelvis must also ensure the function of delivery. An osteometric approach is described to determine the somatic sex from the pelvic bones of a person, the basis of which is the use of the instrumental method of determining the properties of objects, fixed in numerical values. It has been shown that almost all bones of the skeleton have the signs of sexual dimorphism, however, they are most clearly expressed in the bones of the pelvis and skull. Determining the gender of bone remains requires a complete examination of the bones presented. It depends on the number of objects, their state, the completeness of the methods used, as well as on the preparation of the expert. A careful analysis of the greatest number of signs revealed on the bones will allow us to arrive at the most reliable result, which is important when establishing the identity of an unidentified corpse.

Determination of anterior cruciate ligament attachment with radiographic quadrant method to analyze complications following anterior cruciate ligament reconstruction

Summary:

Objective: to demonstrate the potential of radiographic quadrant method for defining the correspondence of the administered femoral tunnel to the anatomical anterior cruciate ligament attachment. Material and Methods. We have surveyed 18 patients aged 23 to 36 yo. (mean age 26±2 yo.) who had underwent ACL arthroscopic transtibial plastic reconstruction in 2012-2014, and had post- surgical ACL graft rapture or knee joint instability. All patients had their knee joints X-raid in standard projections. Bernard and Hertel radiographic quadrant method was employed to take lateral view pictures for defining the accuracy of the administered femoral tunnel. Results. In all cases the sites of the administered femoral tunnels did not match ACL anatomic attachments, defined with Bernard and Hertel grid. Conclusion. Radiographic quadrant method of defining ACL anatomic attachments can be used for post-surgery complications analysis.

Summary:

Purpose: to generalize and systematize the information available in the literature about the anatomy of the posterior connective arteries in adults. Scientific works (monographs, articles, dissertations and abstracts of dissertations) of native and foreign authors for the period from 1928 to 2017 were worked out to achieve the goal. Data on the variants of artery discharge, areas of blood supply and major anomalies are presented. The size characteristics of arteries (length, outer diameter, wall thickness and lumen diameter) are presented taking into account bilateral, age and sexual differences.

Biomolecular assessment of renal function in various types of surgical treatment of renal cell carcinoma

Summary:

Objective: to assess the possibility of using the markers of acute kidney injury to predict the preoperative risk for the subsequent decline in glomerular filtration rate in different types of surgical treatment of renal cell carcinoma (RCC). Material and methods. 60 patients with histologically confirmed RCC T1-3M0N0 operated in the clinic of urology of Saratov State Medical University n.a. V. I. Razumovsky in the volume of nephrectomy or partial nephrectomy. Before surgery all patients underwent standard examination intended for patients with kidney tumors: ultrasound, MRI, excretory urography and dynamic renoscintigraphy, the perioperative values of serum creatinine and glomerular filtration rate were determined. Using the method of immuno-enzymatic analysis the concentrations of excreted with the urine NGAL and IL-18 in serum samples at the preoperative stage, after 5 days and after 1 month of postoperative follow-up were investigated. Differences in clinical data and clinical variables were compared by using Spearman rank correlations and t-test. Results. The determined parameters of acute kidney injury markers IL-18 and NGAL in the early postoperative period were increased in patients after open nephrectomy. Moreover, the correlation analysis according to the Spearman method revealed a strong significant correlation between the preoperative levels of IL-18 and GFR after surgery (r=1; p<0.05). Conclusion. Laparoscopic resection of RCC is the method of choice for surgical treatment of RCC. As a predictor of adverse prognosis, the level of IL-18 in serum may be used. Its increase correlates with a decline in renal function in the postoperative period, and according to some reports, IL-18 also is an independent predictor of adverse prognosis in patients with a localized RCC.

Parkinson's disease in Russia: prevalence and incidence

Summary:

The results of the regional studies on the frequency of Parkinson's disease (PD) and the incidence of it in Russia have been generalized, the main factors that determine the quality of the estimates of this disease epidemiological indicators have been identifyd. The article summarizes data from 19 original studies on the epidemiology of parkinsonism and Parkinson's disease in Russia published between 2005-2015. Due to the statistical heterogeneity of the primary results computational analytics was not applied to the data; however, data consolidation allowed to perform a trend analysis of epidemiological indicators. The methodological basis of the majority of studies was medical aid appealabil-ity; two of the studies used door-to-door surveys. One of the studies returned questionably low epidemiological indicators obtained from the medical records, and the rest showed the standardized prevalence of 30.0-139.9/100,000 and incidence of 7.63-21.8/100,000 per year. Contribution of Parkinson's disease to the nosological structure of parkinsonism was >61.3%. Estimate of the number of patients with PD in Russia is approximately 210,000 people. Conclusions are made regarding the prevalence of PD in Russia according to the cross-cutting research on the level of indicators in the Western countries. The prevalence of PD by appealability is 2-3 times less than the prevalence in continuous research, both national and foreign. The incidence of PD, demonstrated in half of the studies, is stable from region to region and is comparable with the universally recognized values.
