Comprehensive comparative evaluation of results of surgical management of unstable distal tibia fractures

Objective: to evaluate the outcomes of various methods of surgical interventions and functional properties of lower extremities in patients with complicated ankle fractures. Material and Methods. Clinical outcomes of 140 surgically managed patients with unstable bimalleolar and trimalleolar fractures were studied. 80 patients were managed with external fixation devices applied by original method (main group) and 60 patients were managed with submersible structures (comparison group). Results. Compared to submersible structures, the use of transosseous metalosteosynthesis has led to 100% ankle fracture unions. In early postsurgery period the index of average anatomic and functional result reached values of 93.4% corresponding to "good" and 6.6% corresponding to "satisfactory". Conclusion. It has been established with rheovasography data that transosseous osteosynthesis contributed to restoration of blood microcir-culation in the damaged segment of lower extremity in 8 weeks period after surgery and submersible structures in 6 months period.

Application of external fixation devices for fractures of the femur in modern practice

Diaphyseal fractures of the femur are rather common. In the treatment of fractures of the femur various methods of osteosynthesis can be used: intramedullary, of plate, transosseus. In our country external fixation using external fixation devices has traditionally been popular. Objective. The analysis of the frequency of use of transosseous osteosynthesis in the treatment of fractures of the femur and the identification of the most common indications for the use of the method in the present practice. Materials and Methods. The analysis of the structure of external fixation in fractures of the femur, on the experience of treatment of 126 patients with diaphyseal fractures of the femur. Results. The results of the study revealed that the structure of the osteosynthesis of fractures of the femur transosseous osteosynthesis with the use of external fixation devices is not considered to be common and is used as an alternative method of treatment. Conclusion. The treatment of fractures of the femur using transosseous osteosynthesis is currently used mostly for open, comminuted fractures and revision osteosynthesis, as well as a high risk of infectious complications in the zone of fracture, due to complexity of the method, the lowest strength of fixation and a relatively high percentage of non-specific complications.

The aim of the given study is improvement of surgical treatment results of diaphysial fractures of forearm bones using suggested methods. The analysis of the treatment results of 120 patients with diaphysial fractures of forearm bones has been carried out. Methods of surgical treatment of diaphysial fractures of forearm bones using bar and spoke-bar apparatuses for external fixation have been applied to 60 patients. The results of the research have shown the significant decrease in postoperative complications and shortening of period patients stay in hospital. This allows to obtain good anatomical and functional results

The apparatus and the technique of osteosynthesis ofdiaphyseal fractures of shin bones are presented in the article. The technique combines the possibility of bone fracture fragments position control at compression and reduction in process of cyclic loads. This treatment technique provides the optimization of reparative osteogenesis conditions, earlier restored support-motor functions of an injured limb and prevents contamination of tissues which surround transosseous elements. The constructive solutions of pin-related osteosynthesis presented in the article are adapted in topographical anatomy aspect according to the location of fractures. The technique has been applied to 124 patients with diaphyseal shin bone fractures. Positive outcomes have been recieved in 99,6% of clinical observations.
