Summary:

Purpose: to determine the efectiveness of above-threshold (focal) laser coagulation and micropulse laser exposure (MPLV) in patients with central serous chorioretinopathy (CCA). Material and Methods. 91 people with a diagnosis of CCA were examined. The median age was 35.2 years. Patients of the 1st group (n=19) was performed MPLV a wavelength of 810 nm; group 2 (n=31) — about threshold photocoagulation with the wavelength of 810 nm. Control group — 50 people with a natural course of the process. Results. After 1 month after treatment in group 1, visual acuity increased from 0.5±0.08 to 0.8±0.03. According to optical coherence tomography (OCT), the neuroepithelial detachment decreased from 446 to 341.3 MK. With the chronic course of the process after laser treatment, recurrence of the disease was revealed, which required an additional session of MPLV. In group 2, after 1 month, visual acuity increased from 0.6±0.20 to 0.8±0.03, neuroepithelial detachment decreased from 365 to 269 MK. Relapse of the disease in chronic form required an additional session of MPLV. After 3 months, both groups showed complete adhesion of the neurosensory retina and increased visual acuity to 0.9±1.0. Conclusion. Laser coagulation (above- threshold laser coagulation, micropulse laser exposure) is minimally invasive and highly efective in the treatment of central serous chorioretinopathy. The choice of laser treatment method depends on the

Tactics of delivery and diagnostic evaluation in pregnant women with peripheral vitreoretinal dystrophy

Year: 2019, volume 15 Issue: №2 Pages: 528-532
Heading: Ophtalmology Article type: Original article
Authors: Popova N.V., Goydin A.P., Fabrikantov O.L., Goydin D.A.
Organization: S. Fedorov Eye Microsurgery Federal State Institution, Tambov State University n. a. G.R. Derzhavin
Summary:

Purpose: to develop recommendations on delivery tactics based on the analysis and terms of detection of various forms of peripheral vitreoretinal dystrophies in pregnant women with myopia. Material and Methods. 44 women were examined with a mean age of 31 years. Peripheral vitreochorioretinal dystrophy was detected in 82 %. All patients underwent the following studies: visometry using device TOPCON CV-5000 (Japan), autorefractometry using TOPCON KR-8900 (Japan), ultrasound biometry using TOMEY AL-3000 (Japan), eye fundus was examined by Goldman three-mirror lens using the slit lamp TAKAGI SM-70N. When severe peripheral vitreochorioretinal dystrophy is detected, it is recommended to perform peripheral laser coagulation using laser device IRIDEX IQ-810 (USA). Results. In most cases (68 %) peripheral retinal laser photocoagulation was performed with subsequent recommendations on the possibility of natural delivery, and in 32 % of cases caesarean section delivery was recommended to these women. Conclusion. Decision on delivery tactics in pregnant women with myopia should not be determined by the degree of myopia, but by the severity of pathological changes on the periphery of the retina. Medical examination of pregnant women with obligatory examination of the eye fundus with mydriasis medicamentosus in 30–34 weeks allows to identify patients at risk of retinal detachment and conduct early treatment, thereby initially reducing the frequency of operativedelivery according to the ophthalmological indications.

Epidemiological characteristics of rhegmatogenous retinal detachment in the Central Black Earth region

Summary:

Aim: to estimate the frequency of rhegmatogenous retinal detachment among the population of the Central Black Earth region. Material and Methods. A retrospective study on the study of patients with myopia, who turned to the rhegmatogenous retinal detachment (RRD) in "S. Fedorov Eye Microsurgery Federal State Institution" in the period from 2006 to 2017 The epidemiological characteristics of patients who have applied to the RRF from five regions of the Central Black Earth Region (Belgorod, Voronezh, Kursk, Lipetsk, Tambov) are presented. Results. There is an annual increase in the number of patients with myopia complicated by RRD. The highest number of cases of myopia complicated by RRD was recorded in the age group of 50-59 years and 60 years and older, the smallest — in the age group 0-19 years. The female population suffers from a myopia complicated by the RRD equally with the male, but in the age groups 20-29 years, 30-39 years, men suffer more often, in the age groups 50-59 years, 60 years and older, women suffer more often. Among the patients from the Voronezh region, the largest number of cases of RRD was registered in the age group of 50-59 years, among patients from Tambov, Lipetsk and Kursk regions — in the age category of 60 years and older, from Belgorod region — in the age category of 30-39 years. The majority is represented by pensioners (60.3%), working people — 20.8%, disabled people — 15.8%, others 3.1%. Conclusion. Among the population of the regions of the Central Black Earth region, there is an annual increase in the number of patients with RRD, while the number of such patients will only increase, which indicates the need to improve the organization of eye care to the public.

Analysis of incidence of eye and adnexa diseases in the Central Black Earth region

Year: 2018, volume 14 Issue: №4 Pages: 670-676
Heading: Ophtalmology Article type: Original article
Authors: Osmanov RE, Zhabina UV, Manyakov RR, Osmanov EM.
Organization: S. Fedorov Eye Microsurgery Federal State Institution, Tambov State University n. a. G.R. Derzhavin
Summary:

The purpose is to study the dynamics of general and primary incidence of eye diseases and accessory apparatus among the population of the Central Black Earth region for the period 2008-2017, in comparison with similar indicators in the Russian Federation. Material and Methods. The article presents the data on the dynamics of general and primary incidence of eye diseases as a whole, as well as myopia and cataracts in comparison with Russia and the five regions of the Central Black Earth region (Belgorod, Voronezh, Kursk, Lipetsk, Tambov) from 2008 till 2017 Results. A comparative analysis of morbidity indicators allowed us to identify regions that are unfavorable for ocular morbidity in general and for cataracts and myopia in particular. Conclusion. The total incidence of eye diseases among the population of Russia in 2008 accounted 10994,0±2,6 cases, over ten years there was a decrease to 10370,9±2,5 cases per 100000 persons. The primary incidence rate of eye diseases among the population of Russia in 2008 accounted 3422±1,5 cases, over a ten-year period there was a decrease to 3161,1±1,4 cases per 100000 persons. Among the regions of the Central Black Earth region in the Belgorod region, the indicators studied are higher than the average in Russia. The incidence of eye diseases, in particular, cataracts and myopia tends to decrease. However, its level remains significantly high, which requires the improvement of organization of eye care and the development of measures aimed at preventing the development of eye pathology.

Year: 2014, volume 10 Issue: №4 Pages: 645-648
Heading: Traumatology and Orthopedics Article type: Original article
Authors: Ямщиков О.Н., Марков Д.А., Емельянов С.А.
Organization: Saratov State Medical University, Tambov State University n. a. G.R. Derzhavin
Summary:

Diaphyseal fractures of the femur are rather common. In the treatment of fractures of the femur various methods of osteosynthesis can be used: intramedullary, of plate, transosseus. In our country external fixation using external fixation devices has traditionally been popular. Objective. The analysis of the frequency of use of transosseous osteosynthesis in the treatment of fractures of the femur and the identification of the most common indications for the use of the method in the present practice. Materials and Methods. The analysis of the structure of external fixation in fractures of the femur, on the experience of treatment of 126 patients with diaphyseal fractures of the femur. Results. The results of the study revealed that the structure of the osteosynthesis of fractures of the femur transosseous osteosynthesis with the use of external fixation devices is not considered to be common and is used as an alternative method of treatment. Conclusion. The treatment of fractures of the femur using transosseous osteosynthesis is currently used mostly for open, comminuted fractures and revision osteosynthesis, as well as a high risk of infectious complications in the zone of fracture, due to complexity of the method, the lowest strength of fixation and a relatively high percentage of non-specific complications.
