Technology for optimizing medical decision-making

Summary:

The objective of the study is the development of optimization techniques for making management decisions in healthcare institutions. Material and Methods. We combined the research findings obtained earlier and interviewed two groups of healthcare managers (49 set the optimization principles for making management decisions, 79 tested the technology we designed). Methods of analytical, logical, sociological, content-analysis of official and medical documentation, flow charts, organizational experiment, statistical were used. Results. We established the optimization principles for making management decisions in healthcare institutions: goal orientation, system approach (isomorphism and multiparameter interaction), prospectivity, use of adequate criterial and diagnostic apparatus, flexibility, differentiation, competence, democratization. These enabled designing and testing of the optimization technology guided by the authors' automated multicriterial assessment of successfulness for the studied approach; it provided making and implementing the management decisions that were the most appropriate for achieving results of the medical institution (number of institutions) performance. Conclusion. The technology considered in the study enables setting the trend and implementing goal-oriented measures for optimization of the management decisions in healthcare institutions.

Features of the level and dynamics of early social orphanhood as a medical and social phenomenon in various administrative-territorial formations of the Chelyabinsk region

Summary:

The aim is study the features of the level and dynamics of early social orphanhood as a medical and social phenomenon in various administrative and territorial entities of the Chelyabinsk region. Material and Methods. The method of copying data from the accounting documentation of the maternity hospital was applied (Form No. 32 "Information on medical care for pregnant women, puerperals and parturient women" for 2009-2017, Form No. 96 "Birth history"). A total of 1438 observation units were investigated. Results. During the period from 2009 to 2017 in the Chelyabinsk region there is almost two-fold reduction in the number of early social orphans: from 175 to 84 newborns. The highest prevalence of newborns abandonment was in urban districts:4.5±0.9 cases per 1000 newborns. Among women from rural areas and from megalopolis those indices did not differ significantly: 3.5±0.2 and 3.3±0.4 cases per 1000 newborns respectively (p>0.05). Conclusion. In the Chelyabinsk region the number of early social orphan hood decreased by almost half by the period of 2009-2017. The number of these children in megalopolis, urban and rural districts was subjected to serious fluctuations in the particular years of the study. Considering the differences in the level of birth rate and female population of reproductive age in urban and rural areas it is necessary to use index "prevalence of newborns abandonment per 1000 newborns" proposed by us. After a sharp decrease in absolute and relative values reflecting the number of early social orphans at the turn of 2013-2014, the indicators stabilized in subsequent years. This may be a consequence of both the reduction of preventive measures and the formation in the population of mothers of refusers of a certain contingent, which is insensitive to traditional methods of socio-psychological impact.

Analysis of working conditions of employees of Samara medical institutions

Summary:

Objective: an analysis of the conditions and nature of the work of public health workers in the city of Samara, according to their own research and the results of a special assessment of working conditions for the development of preventive measures. Material and Methods. The study analyzed the results of assessing the factors of the production environment, such as microclimate and illumination in 70 rooms of two large medical institutions of the city of Samara. An estimation of the qualitative and quantitative microbial composition of the air medium in the analyzed rooms was carried out. The analysis of registration cards for a special assessment of the working conditions of workplaces in the same organizations was carried out between 2012 and 2017. Results. In analyzing the conditions and nature of the work of medical workers, it was revealed that the final class of working conditions for doctors and nurses of surgical departments was 3.3, therapeutic 3.2 as a result of biological factors. At the same time, the air in the working area of the study rooms of the surgical and therapeutic sections differed significantly in the structure of the conditionally pathogenic microflora. Physical and chemical factors did not exceed the level of class 3.1-3.2. Conclusion. An analysis of the results of the studies showed the need to optimize ventilation in the residents and nurses, as well as the need to improve the quality of natural and artificial lighting. The expediency of standardization of the conditionally pathogenic microflora of the air environment in class «B» premises is shown.

Organizational and economic parameters of resource provision of medical organizations

Summary:

The aim of the research is to determine the main patterns of activity of in-patient medical organizations according to the resource provision of medical and diagnostic process. Material and methods. 20 medical organizations, including regional organizations, central regional hospitals, Saratov municipal health care organizations, non-private and private medical organizations have been surveyed. The survey of 60 chemists of hospital pharmacies has been conducted. Results. The analysis of the survey has showed the resource provision of medical and diagnostic process by medical products, the degree of technological provision in hospitals of various types and stated the main problems of activity on resource provision. Conclusion. It has been found out that the current medical provision with medical products is connected with the action of considerable number of objective and subjective factors and it defines the efficiency of medical social activity.
