Summary:

Objective: analysis of static-dynamic performance of the musculoskeletal system of athletes of winter sports. Materials and Methods. The evaluation of static-dynamic characteristics of the musculoskeletal system. Results. The highest percentage of load-balancing the body while maintaining a static position was observed in the group of athletes of speed and power of the sport and made up, the lowest — in athletes clearing difficult sport. Significant shift in the distribution of body load were detected in athletes clearing difficult sport in the speed and power sport, none of the athletes of the deviations were found. Conclusion. The survey revealed the features of the coordination ability and load balancing body in athletes of different sports: cycling, hard-house and speed-power.

Summary:

Aim. The authors have developed a comprehensive rehabilitation program promotes the formation of a full-fledged callus after fractures of the lower limbs. Objective: To evaluate the effectiveness of inclusion developed a comprehensive program of rehabilitation in patients with delayed consolidation of long bones. Material and Methods: The study included 10 patients with a diagnosis of fracture in the lower extremity with delayed. Results: Under the influence of a rehabilitation program that includes exercise therapy to individually develop a complex, hydrokinesitherapy, practice on robotic biomechanical complex "KonTreks" exercise on a bicycle ergometer, physiotherapy, was marked by the increase of power and functional parameters of joint and muscular legs. Conclusion: The application of the developed program of rehabilitation treatment of patients with a diagnosis of a fracture in the lower extremities with delayed consolidation is highly effective and pathogenetically substantiated by affecting the speed-strength and power performance of muscles of the lower extremities.

Summary:

The authors describe the method of application of underwater running machine in the treatment of patients with lumbar dorsalgia. Aim: To evaluate the effectiveness of the inclusion of underwater running machine comprehensive rehabilitation program in patients with lumbar dorsalgia to improve the local microcirculation. Material and Methods: The study included 20 patients with lumbar dorsalgia. Results: Application of course of study on an underwater running machine provides patients with lumbosacral dorsopathy pronounced effect on a vascular component. Conclusion: The study demonstrated that the use of an underwater treadmill in the complex treatment of patients with lumbosacral dorsopathy is highly effective and pathogenetically substantiated method factor in the local microcirculation and normalizes physiological static position of the spine.

Aim. The authors described a comprehensive program of frozen shoulder treatment, including extracorporeal shock wave therapy and pelotherapy. Objective: To evaluate the effectiveness of the inclusion of extracorporeal shock wave therapy and pelotherapy in rehabilitation of patients with degenerative diseases of the musculoskeletal system. Materials and Methods: there had been examined 120 patients during the study. Results: The result of the application of complex physiotherapy normalized indicators of metabolic and electrolyte imbalances that are important in the formation of a therapeutic effect. Conclusion: The application of extracorporeal shock wave therapy and in combination with pelotherapy in patients with scapula-humeral periarthritis is the elimination of metabolic and electrolyte imbalance, which is important in degenerative diseases of the musculoskeletal system

Summary:

Aim: to investigate the dynamics of ulcer surface healing depending on type of radiation ulcer and cell product in murine experimental model of severe local radiation injuries after exposure to X-rays. Material and Methods. Mature Wistar rats were used for experiments (males, weight 180-200 grams). Standard model of severe local radiation injuries was used: X-ray irradiation of animals on a modified RAP100-10 device. Isolation of the stromal-vascular fraction carried out by enzymatic treatment of adipose tissue. Autologous cells transplantation performed on day 20 after irradiation in the case of acute radiation injury model (early radiation ulcers) and day 120 in the case of long-term effects of radiation injury model (chronic radiation ulcers). As a positive control allogeneic MMSC derived from rat bone marrow were used. Results. Application of autologous cell products derived from adipose tissue in animal model of severe local radiation injuries was investigated for the first time. It was shown that usage of stromal vascular fraction of adipose tissue for the treatment of early radiation injuries not only leads to better improvement (as compared with the use of MMSCs), but also pronounced therapeutic effect could be observed at an earlier time. On the model of long-term effects of radiation injuries therapeutic effect was observed for all cell products. Conclusion. Cell-based products derived from adipose tissue are promising material for future research and clinical application for treatment of local radiation injuriesK) Described method allows choosing treatment strategy basing on patient's individual characteristics and the type of local radiation injuries.

Summary:

Aim: to study immunogenetic markers of predisposition to the development and protection for Crohn's disease in adults population of the Moscow region. Material and methods. The study included 53 samples of peripheral blood of patients with Crohn's disease in the Moscow region. The control group was represented by 1,700 samples of umbilical cord blood is healthy newborns. Revealing HLA antigens at low level performed by SSO method on DynalRELI 48 processor. The results received with ambiguous interpretation was using PCR-SSP method (Ivitrogen). Results. Were found the positive and negative associations of groups of HLA alleles with clinical form, the course of Crohn's disease and response to steroid treatment, in particular revealed that, predisposition to the development for Crohn's disease in women and with sensitivity to steroid treatment in this disease associated allele group C*12, to the characteristic restricting markers such as Crohn's disease include the В 38 and A*11 markers nonrestricting, nonpenetrating noninflammatory type groups are alleles B*56 and C*14 and C*14 is also associated with the risk of Crohn's disease in men, characteristic markers of protection to the development of the disease crown with chronic relapsing and severe clinical course are DQB1*02 and DQB1*03, respectively. Conclusion. These results demonstrate the need for studies of gene polymorphism HLA-system, not only in relation to the disease in general, but in selected patients with clinical groups.

Summary:

Aim: to compare the repair process of DNA double-strand breaks in mammalian cells after acute versus prolonged exposure to X-ray irradiation with different dose rates. Material and methods. Studies were performed on primary human fibroblasts isolated from skin biopsies of healthy volunteers (women, 29 and 30 years). Cells were irradiated using an X-ray machine RUB RUST-M1 (JSC "Ruselectronics", Moscow, Russia) at 37°C temperature with a dose rate of 400 mGy/min (200 kV, 2*2.4 mA, a filter of 1.5mm AI) or 4 mGy/min (50 kV, 2*0.4 mA, a filter of 1.5 mm AI). Immuno-cytochemical protein staining was utilized for yH2AX and Rad51 foci analysis. Results. Phosphorylated histone H2AX (yH2AX) and the key protein of homologous recombination Rad51 foci formation and disappearance kinetics were investigated simultaneously in primary human dermal fibroblasts after acute and prolonged exposure to X-ray radiation at a same dose. It was shown that the relative yield of yH2AX foci per dose reduces with decrease in dose rate, while the relative yield of Rad51 foci conversely increases. Conclusion. Our findings suggest the fundamental differences in the ratio of non-homologous end joining and homologous recombination DNA repair in acute versus prolonged irradiated cells.

Complex correction of erectile and copulative violations at patients with obesity and reproductive function violation

Summary:

The study aimed the development and assessment of features of corrective action of a medical complex on a lipid imbalance at patients with obesity. Material and methods. For an assessment of features of corrective action of a medical complex on a lipid imbalance at patients with obesity in research I was 50 male patients with obesity and frustration of the reproductive sphere aged from 24 to 68 years are included, middle age was 38,5±6,1 years and 7 healthy persons, men of comparable age without any pathological states, results of which all researches were accepted to values of norm. To all patients included in research, except all-clinical inspection calculation of an index of body weight and the relation of a circle of a waist to a circle of hips, measurement of arterial pressure were applied questioning concerning food and food behavior, anthropometry (growth the body weight, a circle of a waist and hips). Besides all patients conducted laboratory methods the researches including definition of atherogenic fractions of lipids (the general cholesterol, triglycerides, LPNP and LPVP). Researches were conducted before treatment and after a course of treatment. Results. The effective complex program for restoration of reproductive function at patients with obesity is developed. Conclusion. Application of the developed complex program more than its separate components caused the expressed reduction of body weight, mainly due to reduction of fatty tissue and manifestations of visceral obesity in patients with obesity and violation of reproductive function, including due to elimination of a metabolic imbalance.

Clinical results of the complex prevention of the acute postoperative pancreatitis at the surgical gastroenterology

Summary:

The article aims to study the results of the complex prevention of the acute postoperative pancreatitis in the surgical gastroenterology. Material and methods. 2968 patients with various disorders of the digestive system were operated. Empirical preventing of the acute postoperative pancreatitis was used in the control group. Complex prevention of the acute postoperative pancreatitis in the main group of patients included the use of Dalargin, intravenous infusion of Octreotide, duodenal trypsin enzyme inhibition; intraduodenal reversal of pancreatic secret; intraductal injection of Lidocaine and external transnasal drainage of the pancreatic and biliary ducts. Results. The frequency of acute postoperative pancreatitis was 12.2% in the main group. The frequency of acute postoperative pancreatitis was 36.9% in the control group. Increased frequency of a mild form of the acute postoperative pancreatitis observed in the main group compared with the control. At the same time reducing the frequency of the moderate severity and severity forms of the acute postoperative pancreatitis observed in the main group compared with the control. Reduction of the morbidity (13.6% vs. 25.1%), hospital mortality (1.6% vs. 3.5%), the duration of the postoperative hospital bed-day (12.1±0.4 vs. 16.7±0.6) were identified in the main group patients compared with the control group. Conclusion. The use of the given scheme for the complex prevention of the acute postoperative pancreatitis allowed significantly reduce the frequency and severity of illness, morbidity, reduce the duration of postoperative hospital bed-day and hospital mortality, as well as the frequency of both mild and severity, and fatal postoperative complications in all investigated groups of patients.

Summary:

The study aims the influence of development of the acute postoperative pancreatitis at the early postoperative period; determine its influence at the frequency and spectrum of complications after abdominal surgery. Material and methods. The work is based on the results of the complex examination and surgical treatment of 1934 patients with various disorders of the digestive system (complicated duodenal ulcer and gastric ulcer, gastric cancer, the proximal and distal pancreatic cancer, colon cancer and postgastrectomy syndromes). The dependence of the overall incidence of postoperative complications, the number of complications per patient, and the number of infectious and inflammatory complications per patient, hospital mortality and length of postoperative hospital days for the development of acute postoperative pancreatitis were studied. Results. Acute postoperative pancreatitis is a leading cause of morbidity postoperative intra-abdominal operations. 97,8% of the cases of complicated early postoperative period of the operations on the organs of the abdominal cavity caused by the development of acute postoperative pancreatitis. Specific complications for the acute postoperative pancreatitis (satellite complications) were identified. Satellite complication had a clearly defined correlation with the development of the acute postoperative pancreatitis. The negative effect of acute postoperative pancreatitis on the severity of the postoperative period, on the morbidity, on the number of complications per patient, on the number of the infectious and inflammatory complications per patient, hospital mortality and on the duration of the postoperative hospital stay were found.
