Clinical features of psoriasis in patients with chronic viral hepatitis and HIV infection

Year: 2018, volume 14 Issue: №4 Pages: 765-769
Heading: Dermatovenerology Article type: Case report
Authors: Tlish M.M., Kuznetsova T.G., Naatyzh Zh.Yu.

Summary:

Psoriasis is a chronic dermatosis which manifests itself as benign hyperproliferation of epithelial cells with disrupted differentiation and keratinization. In dermatology polymorbidity is quite widespread and relevant, but at the same time it is not sufficiently studied. Psoriasis in patients with HIV infection is often very severe and poorly amenable to standard methods of therapy, which makes study of clinical and immunological specifics of psoriasis natural history in such patients relevant. The disease significantly worsens quality of life, leads to disability and impairs social life of patients. The paper presents a clinical case in which altered clinical presentation and aggressiveness of psoriasis are related to comorbidities, which requires interdisciplinary therapeutic and diagnostic approach.

Summary:

Cases of keratoderma are the most common among genodermatoses and take the second place after a group of ichthyosis. The frequency of occurrence of keratodermia in the world, including Russia, is from 1:300 to 1:15000. There is a very rare form of diffuse keratoderma: Olmsted Syndrome, which in 2009 is described in 10 sources of scientific literature. The defeat not only affects the palms and soles, and skin around the orifices, forms a flexion deformity and con-tracture fingers, until spontaneous amputation. Some observations noted the presence of follicular keratosis of the extremities, leukokeratosis on the face and language, pererastayut joints, alopecia, absence of premolars. We present a clinical case of a patient with this pathology.

Summary:

Neurofibromatosis belongs to the group of systemic hereditary diseases from the group of phakomatosis, characterized by developmental defects of ectodermal and mesodermal structures, mainly of the skin, nervous and bone systems, with an increased risk of developing malignant tumors. There are 7 types of the disease include a wide range of clinical manifestations even within the same family: a progressive course, the involvement of almost all organs and systems in the process, a high risk of complications, including many serious and fatal (cardiovascular) pulmonary insufficiency due to severe skeletal abnormalities, malignant degeneration by neurofibromas, etc.). An analysis of 5 clinical cases with neurofibromatosis is presented.

Clinical case of the Besnier-Boeck-Shaumann syndrome in dermatology practice

Year: 2018, volume 14 Issue: №4 Pages: 755-757
Heading: Dermatovenerology Article type: Case report
Authors: Slesarenko N.A., Morrison A.V., Marchenkna E.M., Moiseev A.A., Roshchepkin V.V.
Organization: Saratov State Medical University
Summary:

Sarcoidosis (Besnier-Boeck-Schaumann syndrome) is a benign systemic disease, which is based on the defeat of the reticuloendothelial system with the formation of epithelioid cell granulomas without caseosa and perifocal inflammation in the absence of Mycobacterium tuberculosis. It occurs in all countries of the world, but the highest incidence is recorded in Northern Europe and the United States. The incidence ranges from 11 to 35 per 100,000 population. The etiology of the disease is unknown. The pathogenesis is based on delayed hyperergic immune reactions, which lead to the formation of granulomas involving T cells, as well as the release of various chemokines and cytokines, including tumor necrosis factor (TNF-alpha). The aim of the work is to present a clinical case, due to the rarity of dermatosis, as well as the complexity of diagnosis.

Psoriasis onset risk assessment in persons with hereditary predisposition with age, stress level and eating behavior taken into account

Year: 2018, volume 14 Issue: №4 Pages: 751-754
Heading: Dermatovenerology Article type: Original article
Authors: Shnaider D.A.
Organization: Saratov State Medical University
Summary:

Purpose: to analyze the significance of stress levels and psychology of eating behavior in people with hereditary predisposition for psoriasis for assessment of psoriasis manifestation risk in the next three years. Material and Methods. Two hundred eighty seven people aged 21-63 years with newly diagnosed psoriasis were investigated. All patients were surveyed using Holmes — Rahe Stress Inventory to assess their stress levels and Dutch Eating Behavior Questionnaire to determine the type of their eating behavior. Results. It has been established that external and emotional eating behavior in people with hereditary predisposition to psoriasis is an unfavorable prognostic factor for manifestation of dermatosis at an earlier age. Conclusion. In persons with hereditary burden index over 0.6 units psoriasis can manifest with stress level below 150 points, as determined by Holmes — Rahe questionnaire, while in persons with a hereditary burden index below 0.2 units disease manifestation is largely associated with a stress level over 300 points.

Summary:

Purpose: to evaluate the effect of medical and social factors on compliance to recommendations for prevention of psoriasis in affected patients. Material and Methods. Two hundred eighteen patients aged 22-43 years with 1 to 10 years from the onset of psoriasis were observed for one year. Patients were surveyed to evaluate their willingness to follow the recommendations for prevention of disease exacerbations after a detailed explanation of these preventive measures. Results. It has been established that men living in rural areas, having a family and with more than 5 years since the onset of psoriasis belong to a social group which has a tendency to ignore most recommendations for prevention of psoriasis. Conclusion. A regression analysis has demonstrated a relationship between degree of compliance to doctor's recommendations for prevention of exacerbations and their annual frequency.

Purpose: evaluation of Pelleve™ method ("Surgitron DF S5" device) effectiveness when combined with application of chitosan-based hydrogel. Material and Methods. A study of cosmetic chitosan-based hydrogel in combination with radiofrequency lifting in a group of patients consisting of 18 people aged 30 to 60 years. Results. With chitosan-containing hydrogel applied to the skin before the procedure of radiofrequency lifting, the skin is heated more quickly which effectively reduces working time of the electrode. Conclusion. The technical outcome of the study is an increased efficiency of radio wave lifting achieved through the use of chitosan-containing hydrogel.

The effect of botulinum toxin type A injections on production of sebum in seborrhea

Year: 2018, volume 14 Issue: №4 Pages: 740-744

Summary:

Aim: assessment of sebum production dynamics in patients who received treatment for mimic and static wrinkles in glabella and forehead regions with phase of a menstrual cycle taken into account. Material and Methods. We observed 15 female patients aged 24-38 years who received botulinum toxin therapy. Each BTX-Avial contained 100 units of lyophilized preparation. The solution of BTX-Awas prepared by mixing it with 2 ml of sterile saline. Five intramuscular injections with 2 U of BTX-A per point were made across the forehead, while 3 U injections were done in the glabellar area. The total amount of botulinum toxin used was 19 U per patient. Results. Maximum decrease in sebum production was observed 4 weeks after botulinum toxin injections, and return to baseline was observed at 16 weeks of follow-up. Conclusion. According to the obtained data, it may be stated that botulinum toxin injections affect sebum production with a maximum decrease observed at 4 weeks after botulinum toxin therapy.

Summary:

The skin is the largest organ of the human body and has many functions. Stem cells of the epidermis and hair follicles are necessary for homeostasis and skin regeneration, which leads to the restoration of the integrity and function of the damaged tissue. Currently, we are actively searching for the best therapeutic strategies to restore the barrier function of the skin. Stem cells are recognized as potential candidates for the treatment of numerous skin pathologies. Therapeutic strategies of modern medicine require an understanding of the potential uses of stem cells for treating skin diseases.
