About the influence of non-drug methods of treatment on reducing body weight in patients with metabolic syndrome

Summary:

The research aims to study influence of various medical complexes on a body weight reduction, the main clinical manifestations and metabolic violations at patients with metabolic syndrome. Material and methods. There had been organized a clinical-functional examination and treatment of 80 patients with various manifestations of metabolic syndrome, aged from 25 to 55 years (39,1 years on the average) with prescription of a metabolic syndrome from 2 to 5 years. Result. Medical complexes with application of physical factors are developed and approved, the efficiency was estimated on indicators of a lipid range (triglycerides, the general cholesterol, lipoproteids of low and high density, (3-proteids and aterogenecity coefficient), and also to level of a glycemia, alanin and asparagin transaminases, creati-nine and urea — to define an extent of violation of metabolic processes. Conclusion. The most expressed results on a reduction of body weight, correction of the main clinical manifestations, metabolic violations and lipid imbalance were received at application of a diet, electrostatic massage from the device "Hivamat", multifactorial polymodal hardware influence from the AlfaLedOxyLight-Spa installation, complex influence on the musculoskeletal device from the Hyuber installation and psychological correction from Shufrid system.

Summary:

The study aimed the development and assessment of features of corrective action of a medical complex on a lipid imbalance at patients with obesity. Material and methods. For an assessment of features of corrective action of a medical complex on a lipid imbalance at patients with obesity in research I was 50 male patients with obesity and frustration of the reproductive sphere aged from 24 to 68 years are included, middle age was 38,5±6,1 years and 7 healthy persons, men of comparable age without any pathological states, results of which all researches were accepted to values of norm. To all patients included in research, except all-clinical inspection calculation of an index of body weight and the relation of a circle of a waist to a circle of hips, measurement of arterial pressure were applied questioning concerning food and food behavior, anthropometry (growth the body weight, a circle of a waist and hips). Besides all patients conducted laboratory methods the researches including definition of atherogenic fractions of lipids (the general cholesterol, triglycerides, LPNP and LPVP). Researches were conducted before treatment and after a course of treatment. Results. The effective complex program for restoration of reproductive function at patients with obesity is developed. Conclusion. Application of the developed complex program more than its separate components caused the expressed reduction of body weight, mainly due to reduction of fatty tissue and manifestations of visceral obesity in patients with obesity and violation of reproductive function, including due to elimination of a metabolic imbalance.

Clinical results of the complex prevention of the acute postoperative pancreatitis at the surgical gastroenterology

Summary:

The article aims to study the results of the complex prevention of the acute postoperative pancreatitis in the surgical gastroenterology. Material and methods. 2968 patients with various disorders of the digestive system were operated. Empirical preventing of the acute postoperative pancreatitis was used in the control group. Complex prevention of the acute postoperative pancreatitis in the main group of patients included the use of Dalargin, intravenous infusion of Octreotide, duodenal trypsin enzyme inhibition; intraduodenal reversal of pancreatic secret; intraductal injection of Lidocaine and external transnasal drainage of the pancreatic and biliary ducts. Results. The frequency of acute postoperative pancreatitis was 12.2% in the main group. The frequency of acute postoperative pancreatitis was 36.9% in the control group. Increased frequency of a mild form of the acute postoperative pancreatitis observed in the main group compared with the control. At the same time reducing the frequency of the moderate severity and severity forms of the acute postoperative pancreatitis observed in the main group compared with the control. Reduction of the morbidity (13.6% vs. 25.1%), hospital mortality (1.6% vs. 3.5%), the duration of the postoperative hospital bed-day (12.1±0.4 vs. 16.7±0.6) were identified in the main group patients compared with the control group. Conclusion. The use of the given scheme for the complex prevention of the acute postoperative pancreatitis allowed significantly reduce the frequency and severity of illness, morbidity, reduce the duration of postoperative hospital bed-day and hospital mortality, as well as the frequency of both mild and severity, and fatal postoperative complications in all investigated groups of patients.

Summary:

The study aims the influence of development of the acute postoperative pancreatitis at the early postoperative period; determine its influence at the frequency and spectrum of complications after abdominal surgery. Material and methods. The work is based on the results of the complex examination and surgical treatment of 1934 patients with various disorders of the digestive system (complicated duodenal ulcer and gastric ulcer, gastric cancer, the proximal and distal pancreatic cancer, colon cancer and postgastrectomy syndromes). The dependence of the overall incidence of postoperative complications, the number of complications per patient, and the number of infectious and inflammatory complications per patient, hospital mortality and length of postoperative hospital days for the development of acute postoperative pancreatitis were studied. Results. Acute postoperative pancreatitis is a leading cause of morbidity postoperative intra-abdominal operations. 97,8% of the cases of complicated early postoperative period of the operations on the organs of the abdominal cavity caused by the development of acute postoperative pancreatitis. Specific complications for the acute postoperative pancreatitis (satellite complications) were identified. Satellite complication had a clearly defined correlation with the development of the acute postoperative pancreatitis. The negative effect of acute postoperative pancreatitis on the severity of the postoperative period, on the morbidity, on the number of complications per patient, on the number of the infectious and inflammatory complications per patient, hospital mortality and on the duration of the postoperative hospital stay were found.

Summary:

Aim: to evaluate the correlations between the frequency of the infectious complications of abdominal surgery and the level of immune response to the surgery. Material and methods. The study has been conducted on 53 patients, who were undergone to pancreaticoduodenal resection with lymphadenectomy. Concentrations of interleukin-4 (IL-4) and interleukin-8 (IL-8) in blood serum were studied. Results. The concentrations of IL-8 were higher in patients with complications compared to those without complications. No statistically significant differences in the concentrations of IL-4 were revealed between the studied groups of patients. Conclusion. Concentrations of pro-and anti-inflammatory cytokines can be used to assess the risk of postoperative complications.

. Effect of the venous outflow ways from pancreatic transplant on carbohydrate metabolism after autotransplantation of pancreas in the experiment

Summary:

The aims: to compare the state of carbohydrate metabolism in animals after pancreatectomy with autotransplantation of the pancreatic segment and with organization of the venous outflow in the inferior vena or portal vein. Material and methods. Proximal resection of the pancreas (group 1), pancreatectomy with autologous transplantation of the pancreas and with reconstruction of the venous outflow from the transplant into the inferior vena cava (group 2) and pancreatectomy with autologous transplantation of the pancreas and with reconstruction of the venous outflow from the transplant into the portal vein (group 3) were performed in 45 animals in the experiment. Examining the status of carbohydrate metabolism was performed by intravenous test for glucose tolerance. Results. Primary higher increase in glucose concentrations as compared to the values obtained at the intact animals and its slower decrease have been observed in animals after pancreatectomy with autotransplantation of the segment of the pancreas on iliac vessels (group 2), as well as on the mesenteric vessels (group 3). Higher blood glucose compared to animals subjected proximal pancreatectomy after 40 minutes after administration of glucose was detected in animals undergoing autotransplantation of the pancreas on iliac vessels (group 2) and in animals after autotransplantation of the pancreas on mesenteric vessels (group 3)— 11.82 (11,39-12,26) mmol/l and 10.65 (10,03-11,32) mmol/l, respectively. The glucose concentration in the blood plasma was lower in the animals of groups 2 and 3 below in comparison with the animals in group 1 to 120 minutes of the experiment. Significant differences in plasma glucose concentration between animals of groups 2 and 3 were not found. Conclusion. Significant effects of the ways of organization of the venous outflow from pancreatic transplant on the concentration of the glucose in the blood plasma by the carbohydrate load after pancreatectomy with autotransplantation of the pancreas were not found.

Summary:

Aim: to study the effect features of combined use of pelotherapy and phytocomplex on main clinical symptoms regress and quality of life in patients with knee joint osteoarthrosis. Material and Methods. The study included 96 patients with knee joint osteoarthrosis; the effectiveness evaluation was estimated in terms of American Rheumatologic Association (ARA), the data of visual-analogue scale (VAS), goniometry and laser Doppler flowmetry, indexes WOMAC, Lekena and the index of activity SIA, criteria of life quality questionnaire HAQ. Results. The complex use of pelotherapy and phytocomplex in conjunction with a drug treatment of patients have a more pronounced positive effect on pain (by 35-36%), function of the knee joint (by 26-28%), microcirculation (1.5-2 times) and the quality of life (by 22-25%) compared to treatment with only drug treatment or combined using of ultrasound and drug therapy. Conclusion. The new method of combined use of pelotherapy and phytocomplex leads to the evident regress of clinical symptomatology at patients with knee joint osteoarthrosis, improves locomotor function, microcirculation of knee joints and quality of life.

Objective: the comparative research of phytocomplex phonophoresis rehabilitation efficiency of patients with knee joint osteoarthrosis. Material and Methods, the study included 100 patients with knee joint osteoarthrosis; the effectiveness evaluation was made in terms of American Rheumatologic Association (ARA), the data of visual-analogue scale (VAS) and goniometry, indexes WOMAC, Lekena and the index of activity SIA, criteria of life quality questionnaire HAQ. Results. The complex use of phyto- and ultrasonic therapy in conjunction with an medicamentous treatment of patients have a more pronounced positive effect on pain (by 26-28%), function of the knee joint (by 22-26%) and the quality of life (by 24%) compared to treatment with medicamentous only treatment or combined using ultrasonic and medicamentous therapy. Conclusion. The new method of phytocomplex phonophoresis leads to the expressed regress of clinical symptomatology at patients with knee joint osteoarthrosis, improves locomotor function of knee joints and quality of life.

Summary:

Aim: to study influence of traction therapy in a pulse mode in a complex with electrotherapy on a condition of blood circulation of the bottom extremities and level of a metabolic exchange. Material and methods. There had been examined 120 patients with a lumbar and sacral dorsopathy aged from 22 to 69 years (middle age of 49,5 years) with prescription of a disease from 1 to 5 years, among them men of 34,2%, women of 65,8%. Results. The analysis of effects of various medical methods on a condition of local blood circulation in the bottom extremities showed that the most expressed its compensation is noted at complex application of mechanical pulse traction influence and electrotherapy for patients with a dorsopathy of lumbar and sacral department of a backbone that is confirmed by restoration to normal values of all indicators rheovasogramm. Conclusion. High clinical results of application of the combined medical and rehabilitation complex are based on compensation of local blood circulation that is shown in elimination of deficiency of blood supply due to improvement of a tone of arterial vessels and elimination of venous stagnation, and also due to increase of linear speed of a blood-groove and development of collateral blood circulation. Application of electrical impulse and mechanical traction influences, more at their combination promotes fermentative activity of the systems responsible for a protein exchange that is important for prevention of degenerate and dystrophic process progressing.

Summary:

Application of the listed author physiotherapeutic and psychocorrectional methods of rehabilitation is actual due to the prevalence of a metabolic syndrome at persons of 30 years and older. The research aimed studying of influence of modern comprehensive programs of rehabilitation on a psycho emotional state and expressiveness of a clinical picture at patients with a metabolic syndrome. Methods of an exposure on the patients with metabolic syndrome were innovative technologies of recovery medicine: electrostatic massage with the device "Хивамат" [khivamat], multifactorial poly-modal hardware influence by the AlfaLedOxyLight-Spa installation, complex exposure on the musculoskeletal device from the Hyuber installation, psychological correction from Shufrid system. Results. In all groups essential regress of the complaints had been fixed. The received results were confirmed also by indicators of uneasiness level according to Lyusher's color test.
