Summary:

The article presents cases of two patients which have Sweet’s syndrome, a rare form of dermatosis. The author summarizes data of etiology, pathogenesis and clinical presentation, and then provides differential diagnosis. Clinical observations of patients with Sweet’s syndrome and describes possible problems of diagnosis process are displayed.

Summary:

The study presents a relatively rare case of dermatosis. It is PAPA syndrome in a 23 years-old female patient which is inherited as an autosomal dominant mode. The data about the etiology, pathogenesis, clinical picture were summarized, differential diagnosis was led. Problems with the treatment of the disease have been showed.

Effciency of narrowband medium wave (UVB) phototherapy in combination with cytostatic therapy in psoriasis

Summary:

The study presents the results of combined usage of narrowband UVB phototherapy with low — dose methotrexate in treatment of psoriasis. 48 patients with moderately severe and severe psoriasis in the phase of progression accompanied by psoriatic arthritis have been observed. 30 patients were treated with UVB therapy with a 311 nm narrowband in combination with low doses of methotrexate (7.5 mg per week). Photobox Waldmann 7002 K (Germany) with TL-01 lamps which generate radiation in 310 – 315 nm wavelength range with maximum 311 nm wavelength emission. The procedures have been carried out 4 times a week. Methotrexate should be received as tours 7.5 mg once a week, according to Weinstein — Frost scheme under the supervision of laboratory parameters. The treatment course consists of three tours. Treatment has been carried out on a background of traditional therapy.The comparison group received conventional therapy in combination with narrowband UVB phototherapy and nonsteroidal anti-infammatory drugs. Evaluation of the effectiveness of the therapy was based upon dynamic assessment of PASI and DLQI indexes and proved by earlier reduction of clinical symptoms than in the comparison group, which resulted in reduction of absolute PASI and DLQI values. No adverse effects caused by the therapy undertaken have been noted. The results of the study appear to prove safety and effectiveness of such therapy in treatment of psoriasis.

Changing in basic determinants of quality of life in people of working age with chronic skin diseases

Summary:

A comparative assessment of health-related quality of life in working age patients with various chronic diseases of skin was carried out in the given research. Two specifc questionnaires DLQI and DSQL were used. DSQL allows us to distinguish the quality of life in patients with various types of dermatitis using more criteria than the questionnaire DLQI (Dermatology Life Quality Index). There has not a comparative analysis of the usage of these questionnaires in patients with different nosological forms of skin diseases been organized. Aims. The study presents a comparative evaluation of health-related quality of life in patients of working age with psoriasis and atopic dermatitis using two specifc questionnaires — DLQI and DSQL. Methods. The work is based on Saratov State Medical University n.a. VI Razumovsky of Ministry of Health and Social Development of Russia, using the resources of Hematology and Pathology Clinic, Department of Clinical Immunology and Allergy, Department of Skin and Venereal Diseases. To identify the prevalence of disease, employable contingent working at factories with no occupational exposures. An estimating of QoL (quality of life) included interviews with questionnaires written in by patients themselves. The obtained data were statistically processed. Results. Quality of life in patients with psoriasis and atopic dermatitis is signifcantly reduced, even in a period of stable remission. The decline in the quality of life according to the most of scales in patients with atopic dermatitis is more signifcant than with psoriasis. Even non-acute phase of chronic dermatitis (atopic dermatitis and psoriasis) is able to infuence the decrease in productivity at work, has a negative impact on daily activities.

The evaluation of blood fow in the face skin by LDF method

Aims. The research aimed the studying and optimizing of the possibility of measuring microcirculatory of blood fow in human skin by laser Doppler fowmetry (LDF) against the various topographic features of the peripheral blood supply of tissues. Materials and methods. 20 people of the age from 20 to 56 years without signs of skin lesions were examined by the LDF device. The LDF device has two radiation sources in the red light at a wavelength range of 650±30 nm and the near — infrared at a wavelength of 805±50 nm. The radiation was carried out with four receiving fbers arranged at distances of 1, 2, 3 and 4 mm from the illuminating fber. The state of microvasculature was studied at four points of skin — in the middle of the forehead, in the center of the right cheek and the symmetric point on the left cheek and chin area. We calculated the average values of microcirculation, its standard deviation and coeffcient of variation. Conclusions. By varying the distance from the illuminating fber to the receiving fber and the wavelength of incident light the values of the microcirculation in the different areas of healthy skin were estimated by laser Doppler fowmetry method (LDF). Tissue probes, the spatial localization of the LDF signal in the tissues and the effective depth of detection have been estimated by Monte Carlo method. It has been found out during the study that the LDF signal changes are directly related to the structural features of the structure of the microvasculature, the depth and density of functioning capillaries. The given method can be used to diagnose the state of biological object in the normal and pathological conditions, as well as evaluating the effectiveness of the treatment of various dermatoses.

Studying of a wave activity condition and cellular metabolism of tissues in patients with perioral dermatitis

Summary:

Perioral dermatitis is a facial skin disease with insuffciently studied ethiology and pathogenetic mechanisms, being one of actual problems of dermatology. It is a chronic relapsing facial skin disease mainly in women of young and middle age (in men and children meets less often). The disease has an independent clinical picture which is different from rosacea, demodecosis, seborrheic dermatitis, etc. The standard diagnostic criterion is a visual estimation of expression of an infammation on the basis of signs of exudative reaction which has a subjective character. Possibilities of a radiometric method for an objective estimation of a facial skin functional condition and indicators of an intracellular metabolism in patients with a perioral dermatitis were frst studied.

Summary:

The possibility of a non-invasive estimation of a condition of a vascular and extravasated component of microcir-culation in tissues in patients with perioral dermatitis has been discussed in the research. The formation of disorders of a vascular component in microcirculation in patients with perioral dermatitis has been displayed with the help of an active resonant radiometry. It records a superweak electromagnetic radio emission of biotissues refecting a condition of vascular permeability and of a transcapillary exchange of water activity.

Effciency of acne therapy with small doses of systemic retinoids

Summary:

The study aimed to research an effciency and safety of isotretinoin usage in small doses in patients with recurrent acne of moderate severity. Materials and methods 40 patients (18 – 27 years) with acne were examined (25 women). All of the patients have had different therapy earlier. To estimate the severity and prevalence of the disease Acne Dermatology Index (ADI) was used. All the patients were ordered isotretinoin (Roaccutane) at a standard dose of 10 mg per day for 3 months. Concentrations of triglycerides, cholesterin, ALT and AST in blood serum have been researched before and during the isotretinoin therapy. After 3-months isotretinoin monotherapy there was a prospective 6-months observation. There have been 40 healthy people examined in a control group. Results. After 3 months of isotretinoin therapy comedones, papules, pustules disappeared in most of the patients, ADI ran to 0,6±0,01 (р<0,001). Isotretinoin in the whole is tolerable, side effects were minimal. By the end of a 3-months therapy laboratory ranges (AST, ALT, triglycerides, etc.) have become comparable with control parameters. Conclusion. Small doses of isotretinoin can quickly solve the problem of an eruption in patients with acne of moderate severity, prevent a recurrence, minimize side effects risk.
