Summary:

Aim: To evaluate the possibilities of the magnetic resonance tomography (MRT) in diagnostics and identification of neurosyphilis. Materials and methods: A total of 1387 patients with different forms of documented neurosyphilis were examined to study the deficiency of neurological function. The diagnosis of neurosyphilis was confirmed by CSF-study. MRT was performed in 57 cases. Results: Magnetic resonance tomography (MRT) in 6 patients showed no pathology, 16 patients had arachnoid cystic changes and in 16 cases hydrocephaly occurred. Cerebral cortical atrophy was presented in 13 patients, in 12 cases — multifocal changes were visualized, 12 patients had white matter structural changes. Two cases required diagnostics. A 59-year-old man was hospitalized with epileptic seizure, progressive dementia and multifocal neurologic symptoms. A 40-year-old man was examined with recurring episodes of acute cere-brospinal accident and progressing cognitive disorders. Conclusion: MRT could be used in differential diagnostics of neurosyphilis imitating space-occupying lesions of cerebrum and spinal cord, tumors of central nervous system and could help to define the form of neurosyphilis

Organization: Mordovian State University
Summary:

Purpose. The study of some indices of the metabolism in patients with the exacerbation of extensive psoriasis, the investigation of the revealed disorder dependence on the clinical peculiarities of the disease and anamnesis data. Methods. 253 patients with extensive psoriasis from the age of 18 to 72 have been investigated. Some indices of lipid and protein metabolism, medium molecular peptide content, lipid peroxidation and antioxidant protection processes in blood plasma have been studied. Results. Dysproteinemia, the increase of С — reactive protein level, seromucoid, medium molecular peptides, common lipids, triglycerides, the reduction of the level of lipid proteins of high density, the intensification of lipid peroxidation processes and the reduction of antioxidant activity have been marked in blood plasma in patients with the exacerbation of extensive psoriasis. The dependence of some indices of metabolism on the clinical picture and the severity of skin process, age, family history has been revealed. Conclusion. Metabolism disorders in patients with psoriasis are of systemic character. It seems appropriate to carry out a deep study of metabolism indices in patients with psoriasis with the following correction of the revealed disorders

Organization: Saratov City Clinical Hospital №7, Saratov State Medical University
Summary:

The research goal is to study the composition of biological vascular endothelial injury markers in dynamic development of burn disease. Materials and methods: the investigation included 32 patients with thermal burns. The markers of vascular endothelial growth factor (VEGF), monocyte chemotactic protein-1 (MCP-1) and the number of circulating endothelial cells (CECs) were observed. Results: it was revealed that levels of VEGF, MCP-1 and CECs were significantly increased in patients with thermal traumas. Conclusion: VEGF and MCP-1 were proved to be early and prominent vascular endothelial injury markers in burn disease

Summary:

Objective: To develop the optimum scheme of acupuncture, aimed at improving the functional status of the visual analyzer in patients with myopia. Materials. Treatment was conducted in the main group by reflexology courses in 10 procedures and basic therapy in the control group, only basic therapy. The examination included the classic ophthal-mologic research methods, ultrasound and Doppler echoofthalmografy with color mapping of blood vessels — the ophthalmic artery (OA), central retinal artery (CRA) and posterior long ciliary arteries (ZDTSA), the study of visual efficiency and visual assessment of the subjective expression of asthenopia. The results of these studies were subjected to statistical analysis using the method of variation statistics with the computation of the arithmetic mean (M), the average error of the arithmetic mean (m) and the level of significance (p). Differences between mean values were considered significant if the probability of errors on the distribution of Student»s less than 0,05. The result showed a pronounced effect method of acupuncture on the clinical features, hemodynamic and ergonomic eye, the severity of visual fatigue and general condition of patients. Conclusion. The use of corporal-auricular acupuncture technique in patients with myopia allowed to increase the visual acuity without correction in 1,6 times, visual acuity with correction in 1,2 times, lower rates of subjective and objective refraction in an average of 29.8 and 32.4%, increase rates of accommodation of more than 1,5 times, to increase blood flow velocity characteristics in the GA by an average of 9.7, the CAC at 33.4 and ZDTSA by 27,5%. All this is possible to reduce the severity of subjective indicators of asthenopia in 58% of cases and to increase visual efficiency by 11,3%

Homocysteine as a risk factor of ischemic heart disease development (review)

Year: 2011, volume 7 Issue: №3 Pages: 619-624
Heading: Internal Diseases Article type: Review
Authors: Skvortsov Yu.l., Korolkova A.S.
Organization: Saratov State Medical University
Summary:

Homocysteine is a product of methionine metabolism. Increased levels of homocysteine strengthen oxidizing stress, disturb endothelial function, raise arterial blood pressure and lead to thrombus formation. Homocysteine increases the risk of development of atherosclerosis, coronary heart disease, cerebral and peripheral vessels diseases. By severity it can be compared with smoking and hypercholesterolemia. High levels of homocysteine cause the development of cardiovascular diseases that occur in people who have never suffered from heart attacks. Vitamin B-12 and folate decrease the homocysteine level and convert it into cysteine and methionine

Summary:

Aim: To estimate the dynamics of intima-media complex in elderly patients with arterial hypertension. Materials: 179 elderly patients with arterial hypertension were involved in the study. Mean intima-media wall thickness (IMT) of common carotid arteries in plaque-free sites and prevalence of plaques were evaluated by B-mode ultrasound investigation (Philips Envisor HD, USA). Results: IMT changing was of nonlinear character, remained stable up to 74 years. Mean rate of the following IMT augmentation was 0.157 mm per year. Frequency of atherosclerotic plaque revealing was significantly increased since the 7th decade. Significant correlation between IMT and systolic, diastolic, mean blood pressure levels or medication spectrum was not revealed. Conclusion: Received data proved significant influence of aging upon IMT enlargement

Summary:

The research goal is to study the correlation between indices of variational pulsometry, entropy of heart rate, temporal and spectral analyses and heart rate variability in healthy patients and patients with coronary heart disease. Materials. Of the study are indicators of variational pulsometry and temporal and spectral analyses from 111 patients with coronary heart disease and in 61 healthy individuals. Results. Most parameters of HRV correlated with each other. In healthy patients the greatest independence was characterized by Mo and LF/HF, in patients with coronary artery disease — only by LF/HF Significant correlation with SDNN RMSSD, TP, LF, HF was determined. Conclusion. The variational pulsometry, temporal and spectral analyses of HRV and entropy of heart rate provide HRV assessment. The indices are interdependent and indicate interchangeability of methods
