Summary:

The analysis of the treatment results of 319 patients with complicated forms of gastroduodenal ulcer disease has been carried out. The group is formed by the 223 patients over 60 years old and 96 young and middle-aged patients. The description ofpreoperative and postoperative treatmentwith application of the controlled sympathetic medication denervation of the elderly and aged people with complicated forms of gastroduodenal ulcer disease in the preoperative period and the exposure of the abdominal nerve plexus to alternative magnetic field of “Magniter" device have been provided. . The treatment algorithm of elderly and aged people with such pathology has been introduced. The results of the research have revealed reliable decrease in postoperative complications development incidence, postoperative complication development relief, earlier improvement of the clinical, laboratory and instrumental indices in elderly and aged patients with complicated forms of gastroduodenal ulcer disease, and also postoperative mortality decrease and cuts of the patients' stay period in hospital.

MEASUREMENT OF BLOOD FLOW IN ARTERIA MENINGEA MEDIA OF PATIENTS WITH FREQUENT EPISODIC TENSION-TYPE HEADACHE NOT ASSOCIATED WITH PERICRANIAL TENDERNESS

Year: 2008, volume 4 Issue: №2 Pages: 95-99
Heading: Neurology Article type: Original article
Authors: E.P. Evdoshenko, L.G. Zaslavsky, A.A. Skoromets
Organization: St. Petersburg Medical University named after academician I.P.Pavlov
Summary:

Twenty seven patients with frequent episodic tension-type headache not associated with pericrania! tenderness (TTH) aged from 18 to 69 years old were examined. Arteria meningea media (AMM) and brachiocephalic arteries (BCA) were examined by supersonic power Doppler scanning during the paroxysm of headache and without the paroxysm of headache. Results: Blood flow in AMM of patients with TTH was significantly different during the paroxysm of headache and without the paroxysm of headache (p0.05). The pattern of hypoperfusion in AMM was displayed in two groups during the paroxysm of headache. The increase of average blood velocity (TAmx) in groups of patients with TTH in comparison with the control group.

CLINICAL-DIAGNOSTIC AND PATHOGENETIC ASPECTS OF CERVICAL DIZZINESS

Summary:

Questions of occurrence of cervical dizziness at 190 patients with cervical osteochondrosis, anomalies ofcraniovertebral transition and "switch" trauma of a cervical partof the backbone are discussed. On the basis of the results of complex examination with use of ultrasonic and neurovisualization techniques pathogenetic mechanisms of the developmentof cervical dizziness are discussed.

ACOUSTIC COGNITIVE CAUSED POTENTIALS (Р300)CHANGES IN CHRONIC ISCHEMIA OF BRAIN

Year: 2008, volume 4 Issue: №2 Pages: 86-90
Heading: Neurology Article type: Original article
Authors: D.P. Garin, I.E. Poverennova
Organization: Samara State Medical University
Summary:

Cognitive infringements represent an actual problem of modern medicine. In elderly men they usually develop on a background of a chronic ischemia of brain (CHBI). P300 research in 134 patients was carried out to define CHBI influence on the basic components of the caused potentials. The group of elderly patients with dyscircularencephalopathy I, II, III stages and group of middle-aged persons with presence CHBI and withoutattributes CHBI were considered. The increase of peaks N2, P3 latency, N2-P3 interpeak interval and decrease of N2/P3 amplitude in elderly patients with positive correlation are revealed depending on the stage of dyscircular encephalopathy. Latency increase and amplitude reduction of the basic components acoustic cognitive potential in middle-aged persons directly correlated with the CHBI presence.

Organization: Gastroenterological Centre of Volga Region Federal District of the Russian Federation, Nizhni Novgorod, Nizhni Novgorod Regional Clinical Hospital named after N.A.Semashko, «Medical Diagnostics» Ltd, Nizhni Novgorod
Summary:

15 patients with the endoscopically and morphologically verified diagnosis of the Crohn’s disease and 14 healthy humans at the age of 23- 24 years have been examined. The Crohn’s disease index activity was 235 - 310 units. The possibility highly effective liquid chromatography blood serum use for diagnosis of the disease has been studied. A blood serum dried extract has been investigated on the "Milichrome A 02" chromatograph (Novosibirsk, "Econova "joint-stock company). The chromatograms have been treated with the use of a multilevel cluster analysis with a pathologic three-measured state form, differing from a healthy human. A diagnostic sensitivity and accuracy of the method has achieved 100%.

ENDOTHELIAL DYSFUNCTION IN PATIENTS WITH DISTURBED MYOCARDIAL CONTRACTIVE ACTIVITY OF THE LEFT VENTRICLE IN CHRONIC HEART FAILURE PROGNOSIS

Year: 2008, volume 4 Issue: №2 Pages: 76-81
Heading: Internal Diseases Article type: Original article
Authors: N.A Kosheleva, A.P Rebrov
Organization: Saratov State Medical University
Summary:

The aim of the given study was to define antithrombogenic and vasodilative vascular wall activity in patients with disturbed function of left ventricle (LV), developed after Q-myocardial infarction; and to specify its role in chronic heart failure (CHF) prognosis. 40 patients after Q-myocardial infarction were examined; compare five group consisted of 20 patients. Antithrombogenic (anticoagulant and fibrinolytic) and vasomotor activity of vascular wall were determined. It was revealed that disturbance of systolic function of left ventricle in patients with CHF is accompanied by decrease of antithrombogenic and vasodilative vascular wall activity. Patients with IV functional class of CHF having long term CHF and blood release value consisted < 33%; during six months of monitoring the number of repeated unfavourable cardiovascular events was essentially increased.
