Трансрадиальный доступ при стентировании сонных артерий

Резюме:

Стентирование каротидных артерий (КАС) в настоящий момент является альтернативой каротидной эндар-терэктомии у больных со стенозом сонной артерии. Крайне важным становится уменьшение частоты осложнений при эндоваскулярном вмешательстве. Результаты стентирования постоянно улучшаются за счет внедрения новых систем защиты головного мозга и небольших систем доставки стентов. В течение многих лет бедренный доступ является наиболее распространенным при КАС, в основном из-за быстрого обучения и возможности применения катетеров большого диаметра. Тем не менее, этот доступ не всегда осуществим при атеросклерозе и извитости артерий нижних конечностей, при некоторых анатомических вариантах дуги аорты. Основанием для применения лучевого доступа является попытка уменьшить частоту геморрагических осложнений в области пункции и избежать необходимости длительного постельного режима. Несколько экспериментальных исследований и одно рандомизированное проспективное исследование подтвердили безопасность и эффективность лучевого доступа при КАС. Стентирование сонной артерии с использованием трансрадиального доступа — безопасный и технически осуществимый метод, однако исследований, опубликованных по этой проблеме, немного и совсем нет таких исследований в отечественной литературе. В представленном обзоре литературы отражен и наш первоначальный опыт использования трансрадиального доступа при стентировании сонных артерий.
